25 | Licensing Program Analyst (LPA) Judy Mora conducted an unannounced Case Management inspection due to an incident which occurred on 12/16/21. LPA Mora met with Center Manager Justine Francisco. LPA was guided on a walk through of the facility at approximately 11:35 AM.
The incident that occurred on 12/16/21 was reported to the department on 12/17/21 via telephone. The incident was reported in a timely manner.
On 12/16/21 an intruder entered the facility playground. The facility went on lock down for about 20 minutes to ensure the intruder was off of the premises before ending the lock down. LPA visually observed the playground and the gate that leads to the residential building. The gate leading to the residential hallway does have a latch on the inside of it (playground area). There is no lock on the latch due to the gate being an emergency exit. The intruder was able to enter the building because of the accessibility of the push door on the playground. LPA advised facility staff to change the accessibility of the door, so that the lock handle is on the playground and not the push bar. Per Center Manager, a work order has been placed for this.
LPA will be sent photos of the door once it has been changed.
The Notice of Site Visit (LIC 9213) – was provided and must remain posted for 30 days.
Exit interview was conducted. Appeal rights were explained and provided. |
