25 | ANNUAL INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual inspection to the above facility on 01/21/2022. LPA arrived at the facility at 11:20 AM and met with Karine Galstyan, Licensee who guided analyst on a tour of the facility. Also present during this inspection, were Silvana Isaian and Shoghik Galstyan, Licensee’s Assistants. Per licensee, facility hours are Monday to Friday from 8:00 a.m. to 5:00 p.m.
This is a one story home consists of 3 bedrooms, 2 restrooms, living room, dining room, kitchen, laundry room in the detached garage, detached converted garage (guest house), front yard, and backyard (fenced). There is a fireplace in the living room which has been blocked off to prevent access to children. The children use bathroom in the hallway, living room, dining room, 1 bedroom, and backyard (fenced). Children go to the backyard through the bedroom which leads to the backyard. Per licensee, areas off limits to children and parents include: 2 Bedrooms, 1 bathroom in the master bedroom, front yard, and detached converted garage (guest house). During this inspection LPA did not observe any stairs in the home. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 11:30 a.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room and dining room (daycare classrooms), LPA observed sofas, a dining table and chairs in the ding room, children toys, children tables and chairs and napping cots in the living room. LPA observed all electrical outlets were covered in living room and dining room. LPA observed 11 children in the living room with licensee and her assistants. Next LPA toured one daycare bedroom adjacent to the kitchen and observed two cribs, and toys in he bedroom. Next LPA toured the bathroom in the hallway of bedrooms and observed child proof safety latches on the cabinets under the sink where licensee stores cleaning compounds.REPORT CONTINUES ON NEXT PAGE 1 of 4 |
