At 1:00 pm Licensing Program Analyst (LPA) Roxana Lopez conducted an unannounced case management-initiated inspection to the above facility on 12/21/2023. A COVID risk assessment was conducted upon entry. LPA met with Olimpia Castellanos, Licensee, who guided analyst on a tour of the facility. The purpose of this inspection is due to licensee’s request for a capacity increase on 10/04/2023. Currently, the licensee’s capacity is eight (8). Licensee wishes to increase to a capacity of fourteen (14). Per Licensee, family members residing in the home are 2 adults and 1 children. Per licensee, operation hours will be Monday to Friday 6:00AM to 6:00PM. There was 3 children present upon arrival- one being an infant. A current children’s roster was available for review.
All areas identified on the facility sketch were inspected, including but not limited to, off limit areas. This is a one-story home which consists of 2 bedrooms, 1 bathroom, living room, dining room, kitchen, front yard and back porch (fenced). The children have access to living room, dining room, bathroom, front yard and back porch.
Per licensee, areas off limits to children and parents include 2 bedrooms and kitchen. The LPA toured all areas used by children during this visit. LPA observed a baby gate and or security knob making off limits inaccessible. Areas that will be used by children were inspected for safety, comfort, cleanliness, telephone service (cell phone), ventilation and heating (ceiling fans and wall heater). Detergents, cleaning compounds, medicines, sharp objects and hazardous items that can pose a danger to children are inaccessible in areas designated for children.
