25 | CASE MANAGEMENT INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced case management inspection to the above facility on 11/18/2021. Facility was placed on required inspection for two years on November 2019. LPA arrived at the facility at 11:30 AM and met with Nona Sahinyan, Licensee who guided analyst on a tour of the facility. Also present during this inspection, were Aram Sahinyan, licensee's son, Shushanik Gevorgyan, licensee's mother, Naira Margaryan and Janette Aviles, licensee's assistants. Per licensee, Nona Sahinyan facility hours are Monday to Friday from 7:00 a.m. to 11:00 p.m. Per licensee she is affiliated with Pacific Clinic Head Start Program.
This is a one story home consists of 3 bedrooms, 2 restrooms, living room, dining room, kitchen, front yard and backyard (fenced). There are two entrance doors which leads to the living room and bedrooms hallway.
The children use the living room, dining room, kitchen, bathroom adjacent to the kitchen and backyard (fenced). Per licensee, areas off limits to children and parents include: 3 bedrooms, bathroom in the hallway of bedrooms, and front yard. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 11:45 a.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room and dining room (daycare classrooms), LPA observed sofas, a TV, dining table, chairs, children's table and chairs and toys. LPA observed all electrical outlets were covered in living room and dining room. Next LPA toured the children's bathroom adjacent to the kitchen and did not observe any hazards. The restroom that children use was observed to be safe and sanitary. Next LPA toured the kitchen and observed children safety proof latches on the cabinets where licensee stores sharp items and cleaning compounds.
