Licensing Program Analysts (LPAs) Jeanette Estrada and Christine Nolan conducted an unannounced annual inspection at the facility. LPA met with Principal Kristina Damon and Early Learning Center Manager Keokuk Legarde. LPAs advised facility representatives of the purpose of the visit. LPAs observed a total of 35 children supervised by 18 staff.
This is an infant program which operates Monday through Friday 7:45 AM to 2:45 PM. There are a total 6 classrooms in this infant program: 3 infant classrooms for ages and 3 toddler classrooms for ages There is also a preschool program which operates at the same facility under license number 198019590.
LPAs inspected all operating classrooms and areas used by children. LPAs observed all documents required to be posted in the facility. Furniture and equipment were inspected for age appropriateness and good repair in the classrooms. There are a variety of age appropriate toys and materials available for children. Telephone service, heating, lighting and ventilation were evaluated and are operable. The facility has central air and heating. Lighting was observed to be in operable condition. Children have access to cubbies in the classrooms to store their belongings. Floors of each classroom were observed to be clean and free of hazards. Each classroom has a separate napping area equipped with cribs and napping cots which is supervised by a staff member when in use. Each infant that requires a crib has their own assigned crib. LPAs observed there were no hazards in the cribs. LPAs observed napping cots available for children who have transitioned out of the crib. Bedding is provided and laundered by the program. Staff keep a sleeping log for all infants enrolled. Infant naps are given as needed. Infants who have transitioned to a cot are transitioned to a schedule.
The restrooms were observed to be clean and free of hazards with the necessary items (soap/paper towels/ diapers) provided. There is one restroom in between every two classrooms. During this inspection LPAs inspected a total of 3 restrooms.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

There are two toilets, one child sized sink, two diaper changing tables and one adult sized hand washing sink within arms reach in each restroom. The program provides diapering supplies. There are two additional child sized hand washing sinks in each classroom.
Availability of indoor drinking water was observed in the classrooms via a water fountain connected to the hand washing sink in the classroom. For this program water is available in water jugs and cups with assistance from staff.
Disinfectants, cleaning solutions and medications are stored inaccessible to children in care. In each classroom, cleaning supplies are stored in locked cabinets. Medication is stored in the classroom's first aid backpack. LPA reviewed the medication administration policy with facility representatives. A list of medications and allergies/food accommodations is kept in the classrooms accessible to staff.
Per Facility Representative there are no poisons stored in the facility, there are no bodies of water. LPA observed an operable carbon monoxide detector in the facility and the facility has a centralized fire alarm system. The isolation room is located in the main office area Room 1109.
Sign in/out procedure for this program was discussed. Per Facility Representatives, parents sign children in the classrooms. The program conducts an emergency drill each month. The last documented drill (evacuation drill) was conducted on 6/24/25.
Per Facility Representatives, this program provides meals. Depending on the needs and services plan, infants and toddlers are fed as needed or are transitioned to a regular feeding schedule. Infants who are not ready for table food are provided with infant formula and baby food. Parents have the option of bringing breast milk or their own preferred formula. Children's formula is labeled with the child's name. Breast milk is stored in the facility kitchen and it is labeled and dated. Infants and toddlers who do eat table food are provided with breakfast, lunch and snack. Meals are delivered to the facility kitchen from the main office and distributed by kitchen staff. LPA inspected the kitchen area and did not observe hazards. Menus are posted and provided to the parents every month.
The outdoor area was inspected. The outdoor area is fully enclosed. Each component has a separate designated outdoor play area. LPA observed shaded areas for children and absorbent material under high climbing structures. LPA also observed water fountains available for children's use. Additional drinking water is provided via water containers. All materials and equipment were in good repair. Staff provide supervision by creating and supervising zones for children to play in. The infant program also has buggies with seat belts and shade available for children for additional outdoor activities.

Children’s Records were reviewed for completeness. Children's records contained all required documents and information.
Staff records were not reviewed during this inspection. LPA will review staff records at a later time.
Facility Representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. LPA discussed the safe sleep regulations with Facility Representatives and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms.
