Licensing Program Analysts (LPAs) Nolan Tcheng and Suzana Safaryan conducted an unannounced required inspection to the above facility. At 12:50pm, LPAs met Assistant Marine Sosyan, Licensee of adjacent facility #198018963. Also present during this inspection, was licensee's other Assistant. Licensee Taron Mayilyan arrived at 1:08pm. Per licensee, they stated they were helping get food for meals. The licensee states that they currently has 14 children enrolled. A current children’s roster is available and is current. Licensee’s hours of operation are 7am-11pm Monday-Saturday.

This is a one-story home which consists of 2 bedrooms, 2 bathrooms, kitchen, dining area, living room, side (lunch) room, and backyard (fenced). The children use both bedrooms, 1 bathroom, living room, side room, kitchen, and backyard. Per licensee, areas off limits to children and parent include: the adult bathroom in the hallway. All areas identified on the facility sketch were inspected. The licensee provides food for children in care.

The licensee states that 1 adult and 0 minors currently live in the home. All persons living in the home are identified on the attached LIC811. Licensee states that they currently have one assistant. All individuals present in the home have obtained a criminal record clearance or exemption prior to working, residing or volunteering in a licensed home. Per licensee, there are no weapons, firearms or bodies of water on the premises.

At 1pm, LPAs were led on a tour of the facility by the licensee. Upon entry in to the home, LPA reviewed the living room which is used as the main child care area. Emergency Disaster Plan, Parent’s Rights Poster and the Facility License are observed to be posted next to the front door. There were age appropriate toys and equipment for children in care to use. Fireplaces and open face heaters are inaccessible to prevent access by children. LPA observed cubbies and an area for children to place their belongings. The next room reviewed was the dining area, that had age appropriate chairs and tables for children in care. There are first aid supplies available. REPORT CONTINUES PAGE 1 of 3

At 1:15pm, a fire extinguisher was observed. The valve on the required 2A 10BC fire extinguisher indicates fully charged. Per licensee, they do not know where they placed the receipt. Report from 04/03/2023 states there was a receipt that was dated 11/28/2022. LPA discussed. LPA reminded licensee to ensure that the receipt does not get lost and remains with the canister. Per State Fire Marshall standards, fire extinguishers shall be serviced annually. The tour continued to the side room which is used for lunch or activities. Per licensee, the side room or bedroom will be used as an area for an ill child until an authorized representative comes to pick up child. The kitchen was reviewed. Sharp knives were observed to be kept up high in a cabinet out of reach for children. The tour continued down the hallway, LPA reviewed the licensee's bedroom, first door on the left. LPA observed a play yard and a small bed present. Following the bedroom on the left, was the bathroom for adults only. There was a child safety cover on the door knob. LPA observed toiletries and cleaning compounds in the bathroom unavailable to children. Smoke and carbon monoxide detectors were tested and are operable. The bedroom at the end of the hall was reviewed. There were children napping on small bed/cots in the bedroom. The tour continued to the side hallway where the children bathroom was observed. The licensee states that a cell phone is used and stays at the facility during operating hours.

At 1:35pm, LPAs observed the back yard area. The outdoor play area was observed to be fenced. At this time, children are using the back yard for outdoor play time. The backyard is shared with 524 S. Chevy Chase Dr (#19018963). Per licensee, they use the back yard for play time at 10-11am. The other facility uses the outdoor area at 11am-12pm. LPAs observed playhouses, small slides, carts, bikes, and a couch area with shade for children to relax on. The licensee is observed to be operating within the license capacity limitations.

The licensee and other personnel have completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expired on 08/2023. Licensee states they will enroll in the course and submit the registration to LPA. Licensee has completed mandated reporter training that expired 08/11/2023. During the inspection, Licensee started their Child Care Provider Training. Licensee will submit proof of completion to LPA. The licensee does have proof of immunization against influenza, tuberculosis, pertussis, and measles. Children’s records were reviewed, including but not limited to, a copy of the emergency information card that contains all the information specified by regulation.

The following was discussed:
-There are no pets on the premises.
-Infant Walkers, Johnny Jumpers, Saucer Chairs, and/or any other item that falls into these categories are not permitted in a family child care facility.
-Smoking is prohibited in a license family child care home.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated

Licensee is not caring for infants.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the licensee confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Advisory Notes for Pediatric CPR/First Aid and Mandated Reporter are being issued.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the licensee Taron Mayilyan, at 2:45pm. Copy of report provided.
