At 03/05/2024 , at 01:30 pm , Licensing Program Analyst`s (LPA`s) Shushanik Safaryan and Cynthia Reyes conducted an unannounced required 3-year inspection to the above facility. A COVID risk assessment was conducted upon entry. At 01:34pm LPA`s met with Marine Sosyan , Licensee of adjacent facility #198018963 to whom the reason of the visit was explained. LPAs met the assistant Gayane Chobanyan too Licensee Taron Mayilyan came later around 01:50 pm . Per licensee he was taking lunch .Per Licensee, there are 14 children currently enrolled. There were 12 children present during the inspection .The licensee was observed to be operating within the license capacity limitations.

The licensee states that 1 adult and no children currently live in the home. All persons living in the home are identified on the attached LIC811.

Licensee states that they have currently one assistant . All individuals present in the home have obtained a criminal record clearance or exemption prior to working, residing, or volunteering in a licensed home.

This is a one-story home which consists of 2 bedrooms, 2 bathrooms, living room, dining room, side room ,kitchen and backyard which is fenced.

Care is provided in the living room, dining room, 2 bedrooms, 1 bathroom and backyard.
Per licensee, areas off limits to children and parents include :the adult bathroom in the hallway, kitchen , side room .Sketch was updated .
Parents enter the home through the main entrance which leads to the living room where the main care provided .
During this visit, all areas identified that are accessible for children to use were inspected for the safety, comfort, and cleanliness.

At 01:38 pm Marine Sosyan , before licensees’ arrival guided LPAs to the tour of the home. Facility License, Emergency Disaster Plan, Parent’s Rights Poster, and Earthquake Preparedness Checklist were observed to be posted in the living room next to the entrance .A current children’s roster was available for review.

LPAs were toured starting from the main entrance which leads to the living room where LPAs observed age-appropriate toys and equipment for children in care . Fireplace and open face heater are screened to prevent access by children. LPAs observed cubbies as an area for children to place their belongings. The next room reviewed was the dining room , that had age-appropriate chairs and tables for children in care. There are first aid supplies available to children in care . The tour continued to the side room which is used for lunch and will be inaccessible to children in care .LPAs observed child safety cover on the doorknob . The kitchen was reviewed. Sharp knives were observed to be kept up high in a cabinet out of reach for children. The tour continued down the hallway, LPA reviewed the licensee's bedroom, first door on the left. Per licensee his bedroom will be used to isolate ill child . LPA observed a play yard and a small bed present. Following the bedroom on the left, was the bathroom for adults only. There was a child safety cover on the doorknob. LPA observed toiletries and cleaning compounds in the bathroom unavailable to children. Smoke and carbon monoxide detectors were tested and are operable. The bedroom at the end of the hall was reviewed . There were 12 children napping on small bed/cots in the bedroom. The tour continued to the side hallway where the children bathroom was observed.
There is telephone service via a cellphone that stays at the facility during operation hours. Hours of operation are Monday – Friday, 07:00 am – 11:00 pm. There is central air and heating in the home.

Detergents, cleaning compounds, and medications were observed to be inaccessible to children. Per Licensee, there are no poisons in the home. LPAs advised that any poisons should be locked under key or combination lock. Licensee states that there are no firearms or weapons stored in the home.
LPAs observed 2 required fire extinguishers in the home .The valve on the required 2A 10BC fire extinguisher indicates fully charged . Per licensee it was purchased in September . LPA`s reminded licensee to keep the receipt available for the departments review .

All homes shall conduct fire and disaster drills at least once every six months and document the date and time of each drill. Last drill documented was conducted on 03/01/2024.

The home is observed to be clean and orderly. There are age-appropriate toys available for children.

Per licensee, he provides lunch and am , pm snack for children in care.

At 01:45 pm LPAs observed back yard area .Currently, children are using the backyard for outdoor play time. The outdoor play area was observed to be fenced and gated. The backyard is shared with 524 S. Chevy Chase Dr (#19018963). Per licensee, they use the back yard for play time at 10am -11am. The other facility uses the outdoor area at 11am-12pm. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA did not observe any objects that can pose danger to children. No bodies of water were observed on this day.



At 02: 30 pm Licensee and staff records were reviewed. The licensee has completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 09/2025.

LPA observed that the Licensee does have proof of Mandated Reporter AB 1207 compliant Child Care Training Certificate on file with expiration on 09/2025. www.mandatedreporterca.com

The licensee does have proof of immunization against influenza, tuberculosis, pertussis, and measles.

At 03:00 pm children’s records were reviewed, including emergency information and were observed to be complete.
Licensee states he is currently not caring for any infants.

LPA created an LIC 857, Children’s Record Review, and LIC 859, Staff Record Review which documents staff and children’s files reviewed during this inspection.
Per licensee, there are no animals on the premises.
Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home.

LPAs did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs, and/or any other item that fall into these categories are not permitted in a family childcare facility.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California

During the exit interview, the LICENSEE Taron Mayiyan confirmed that there are no Registered Sex Offenders living in the facility .

Currently, the licensee is in compliance with California Code of Regulations Title 22. No deficiencies cited.

A notice of site visit was given and must remain posted for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
