On January 15, 2025, at 12:20 pm, Licensing Program Analyst (LPA) A. Carter conducted an unannounced Annual/Random Inspection and met with Facility representative (FR), Daisy Ramirez. LPA disclosed the purpose of the inspection and was granted entry into the facility. FR was provided a copy of the Facility Entrance Checklist.
LPA was taken on a guided tour of both the indoors and outdoors of the facility. Teacher-child ratios were observed, and staff names recorded. All center staff that were present during today’s inspection had fingerprint clearance and are associated to the designated license number. Care and supervision were evaluated to determine if the basic needs of children are met and appropriate. The sign-in and out sheets were reviewed. Personal Rights of children were observed by LPA.
The hours of operation are 7:00 am – 5:30 pm, Monday - Friday. Facility is currently licensed for a capacity of 33. At the initial start of the site visit, there is a total census of 27 children and a total census of 5 teachers present. The facility is made up of 3 classrooms. Room 1: Creators (4-5yr olds), Room 2: Explores (2yr olds), and Room 3: Imagineers (3yr olds). All classrooms were inspected for cleanliness and good repair. There were age appropriate toys and equipment. LPA observed all posting requirements for operation on the posting board and the following documents were observed: LIC 203A-License, LIC 610 A-Emergency Disaster Plan, LIC 9148-Earthquake Preparedness Check List, PUB 394-Notification of parent’s rights poster, LIC 613A- Personal Rights, PUB 269- Child Car Seat Law, Menus, and Activity Schedule.

Furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Children napping equipment met the requirement regulation at time of inspection. Storage for children's belongings, bathrooms facilities, and isolation areas were inspected. The isolation area is inside the directors office. The facility has two restrooms with two age-appropriate toilets and sinks. LPA inspected all restrooms for availability, good repair, water temperature, toilet paper, safety, and sanitation.

Each classroom has cubby/storage area for children’s personal belongings. LPA observed puzzles, books, and other appropriate toys to be clean and free of hazards at time of inspection. Age-appropriate tables and chairs in each classroom was observed by LPA. The main first aid kit located across from the restrooms and small first aid box kept in each classroom was observed by LPA. First Aid supplies were inventoried and in compliance with regulation.
Menus were reviewed at time of inspection. Facility provides breakfast, lunch, and snacks. Lunch is prepared at the Buena Park site and delivered to the facility. Breakfast and snacks are prepared in the facility kitchen by staff. The food preparation area was toured and found in compliance for safety, cleanliness, and proper storage. Drinking water is provided via water pitcher with disposable cups in each classroom for children in care. LPA confirmed Carbon monoxide and smoke detectors were present and determined to be in operable condition. Fire extinguishers were observed as serviced on 10/21/24. Hazardous items including poisonous cleaning compounds were stored in the kitchen cupboard locked and inaccessible to children. A review of medication policy, including administering, labeling, storage, and records was made. Per FR, there is one child (C4) with medication that is stored in director’s office.

At 12:30 pm, outdoor equipment was inspected for safety, cushioning material, good repair, and age-appropriateness. LPA observed tricycles, sensory tables, play structures, and other age-appropriate toys around the play area. LPA observed play area to have required shade and fencing. The children have access to drinking water via water pitcher and disposable cups from classrooms. Play area was inspected for hazards and inaccessibility to bodies of water; no bodies of water or hazards observed at time of inspection.

At 12:45 pm, staff and children’s records were reviewed. Mandatory forms for the children and staff files were discussed. LPA observed current Adult / Pediatric CPR and First Aid Certifications with Epinephrine Auto-Injector Administration and Asthma and quick relief medication administration for center staff. CPR card expires on 09/12/26 for all staff members. Role and responsibilities of being a Mandated Reporter were reviewed and training for all stall completed on 10/11/23. FR was advised how to access forms and Regulations online at www.ccld.ca.gov. to ensure the most current information regarding regulations for all those who assists in providing care.
LPA advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to CCL within the time frame specified by the regulation, this includes any changes as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your location.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.
