25 | REQUIRED- 1 YEAR INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced required 1- year inspection to the above facility on 09/12/23. LPA arrived at the facility at 1:05 PM and met with Melanya Davtyan, Licensee who guided analyst on a tour of the facility. A COVID 19 risk assessment was conducted prior to entering the facility. Also present during this inspection, was Alla Antonyan, Licensee’s Assistant. Per licensee, Melanya Davtyan facility hours are Monday to Friday from 7:00 a.m. to 10:00 p.m. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensee.
All areas identified on the facility sketch were inspected. This is a one story home located on the first level. The home consists of 3 bedrooms, 1 & 1/2 restrooms, living room, dining room, kitchen, garage, front yard and backyard (fenced). Per licensee the children use the living room, dining room, two bedrooms, half bathroom in the bedroom, kitchen, and backyard (fenced). Per licensee areas off limits to children and parents include: 1 bedroom, one bathroom in the hallway, garage, front yard, and back of backyard. LPA did not observe stairs in the home. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report. Per licensee, children and parents enter the home through the side gate which leads to the backyard and den bedroom or main entrance.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 1:20 p.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room and dining room (daycare classrooms). LPA observed all electrical outlets were covered in living room and dining room. There is a fireplace in the living room which has been blocked off to prevent access to children. LPA observed a TV, children tables, chairs, and toys in the living room and dining room. LPA observed a safety gate was installed to the kitchen. Next LPA toured the one bedroom adjacent to the dining room and observed 5 children beds in the bedroom and five children were getting ready to take nap.
