Licensing Program Analyst (LPA) Veronica Martinez Garza conducted an unannounced Annual/Random inspection at the above facility on 05/06/24 at 09:05 a.m. LPA met with Director/Owner, Celine Chouteau who guided LPA on a tour of the facility. This is a preschool age program licensed for 26 children. Facility operates Monday – Friday from 08:45 AM – 04:30 PM. Per the director, there are 27 children enrolled.

All areas identified on the facility sketch were inspected. This is a two-story home that has been granted permission by the City of Los Angeles Department of Buildings and Safety to operate as a Child Care Facility. The following staff was present during this inspection: 07 children with 02 staff in group A1, 08 children with 01 staff in group A2 and 08 children with 01 staff in group B. Indoor classrooms used are: Classroom 1 (Library), Classroom 2 (Main Area), Classroom 3 (Science Area). The second floor was observed to be made inaccessible to children by a security gate located at the bottom of the stairs. Teacher-child ratios were observed to be in accordance with Title 22 Regulations. Staff names were recorded on the attached LIC859. All children were observed to be under supervision, including visual supervision, of a teacher at all times. The licensed facility is within the conditions, limitations, and capacity specified on the license.


Upon arrival, LPA observed children and parents enter through the side entrance/driveway. LPA observed the gate is rusted and has a metal strip sticking out. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. Indoor furniture and equipment are in good condition, free of sharp, loose, or pointed parts. Drinking water was readily available indoors and outdoors. According to the director, children bring their own water bottle and is refilled as needed by using Alkaline water from the kitchen faucet. LPA observed cots are used as napping equipment.

LPA observed all children have their own cubby. All toilets and hand washing facilities are safe and are operating properly. LPA observed the children's bathroom not tidy. At this time, the "library" is used as an isolation area. A cot is available for an ill child to rest on. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. The facility was observed to be free of flies, other insects, and rodents.

LPA observed LIC 610, LIC 9148 and Emergency disaster drill posted in the kitchen of the facility. The facility does not have a parent board. LPA observed a current Emergency disaster drill. Last drill conducted on 05/02/24.Snack menus were reviewed to ensure that they are being posted at least one week in advance and visible to an authorized representative. Snacks were reviewed for availability, quantity, and appropriateness to children in care. Facility provides AM/PM snack and lunch. All food is protected from contamination. Director states there are no weapons, firearms, or bodies of water on the premises.

First Aid supplies are available in kitchen and outdoors. LPA observed a functioning carbon monoxide detector. LPA also observed a fire extinguisher in the kitchen indicating fully charged and last service date 09/08/23.

At approximately 09:35 a.m., LPA took a tour of the outdoor play yard. According to the director, this facility is mainly outdoors. Children eat, play, and explore in the play yard. LPA inspected the outdoor play equipment for any lose or pointed parts. Outdoor areas around and/or under climbing equipment have cushioning material to absorb a fall. The facility has 2 areas with sand, 1 being a sand box. There is adequate shade in the play yard. LPA observed several spider webs throughout the play yard. According to the director, every three months there is an eco-friendly pest control company that comes to the facility and takes care of all spider webs and ants. Also, the cook maintains the play yard. Director provided LPA with a copy of proof of service for the pest control. LPA also observed a playhouse and table that had wood chipping throughout. There was also a chair that according to the director it was missing a cover.

LPA observed the base of a chair that was dirty and did not seem operable. Director removed the chair. There were 2 outdoor faucets for children to wash their hands that were observed to be dirty. LPA advised as advised to check with the American Standard Testing Materials for Playground Guidelines and/or to use the manufactures recommendations for the playground equipment in efforts to be more in line with safety guidelines.

Sign in and out sheets were reviewed to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. The facility utilizes the application called ProCare. LPA observed 23 children were signed in; however, 1 child was signed in by facility staff. Children’s roster was reviewed and was observed to be current.

Children’s Records were reviewed to ensure that Identification and Emergency form and a medical assessment are on file. LPA observed all children’s files are complete.

Staff Records were reviewed to ensure that appropriate documentation of education credits is on file. Criminal Records Clearance for adults and verification of CPR/First Aid and health preventative practices documentation was reviewed. LPA observed Staff #1 (S1) and S4 are missing proof of Influenza vaccine or declination letter. S6 is missing MMR and Tdap vaccines. S2, S3, and S4 are missing proof of the Mandated Reporter Training AB1207. S2 and S6 are missing LIC 503. S2 is missing proof of transcripts.


SB792 (Immunization Requirements for Staff and Employees) was discussed with the director.

AB1207 Mandated Child Abuse Reporting – On or before March 30, 2018, any person who works in a child care facility shall complete the training and renew the training every 2 years. Website provided: https://www.mandatedreporterca.com/training/child-care-providers.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES:

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

The following deficiencies were cited in accordance with Title 22 of the California Code of Regulations and Health & Safety Codes. Please see 809D for documentation of deficiencies.

A notice of site visit was given and must remain posted for 30 days.
