Licensing Program Analyst (LPA) R. Derraco conducted an unannounced complaint inspection to the above mentioned facility on 12/18/24. LPA arrived at the facility at 9:15 AM and was met by Director, Maile Juarez, who guided analyst on a tour of the facility. LPA observed 5 adults caring for children, and 32 children in care. The facility was observed to be clean and in good repair.

The purpose of this inspection is to deliver findings to the above mentioned allegations. During the investigation, LPA made observations, conducted interviews, and reviewed records. Individuals interviewed state that potty training services are provided to children in care. Individuals state that they either request the help of a teacher when using the restroom, or the child in care uses the restroom by themselves. Individuals interviewed state that if an injury occurs with a child in care, parents are contacted through the brightwheel application, and the Director is notified to take the next steps. Next steps include contacting parents and following up with families to determine if medical attention is needed. LPA observed that the Director

understands how to file an unusual incident report. Individuals interviewed state that food is served "family style" and no child is refused food during meal time. Individuals state that when a specialized diet is required, children sit alongside each other or at a separate table. During the investigation, LPA observed a neon pink allergy list / specialized diet list posted in each classroom. Individuals interviewed state when there is a change, the Director usually changes the color of the list and sends a message through brightwheel to advise teachers. One individual that was interviewed stated that they had concerns with the facility, however the concerns did not corroborate any of the above listed allegations. During the investigation, LPA observed the facility to be within teacher to child ratio during complaint inspections (3). Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.

A notice of site visit was given and must remain posted for 30 days.
