25 | REQUIRED- 3 YEAR YEAR INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced required 3 year inspection to the above facility on 05/01/24. LPA arrived at the facility at 11:50 AM and met with Liana Tarverdyan, Licensee who guided analyst on a tour of the facility. Also present during this inspection, were Diana Avagyan and Hasmik Ashughatoyan, Licensee’s Assistants. Per licensee, Liana Tarverdyan facility hours are Monday to Friday from 7:30 a.m. to 10:00 p.m. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensee.
This is a one story home located on the first level. The home consists of 3 bedrooms, 1 & 1/2 bathrooms, kitchen, dining room, living room (FIREPLACE: inaccessible), office, laundry area, garage, front yard and backyard (fenced). Per licensee the children use the living room, dining room, bathroom in the hallway, outside bathroom and backyard (fenced). Per licensee areas off limits to children and parents include: 3 Bedrooms, kitchen, office, laundry area, garage, and front yard. LPA did not observe stairs in the home. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report. Per licensee, parents enter the home through the main entrance which leads to the living room.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 12:00 p.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room and dining room (daycare classrooms), LPA observed a TV, children tables, chairs, toys, and a pack 'n play in the daycare classrooms. LPA observed all electrical outlets were covered in living room and dining room. There is a fireplace in the living room which has been blocked off to prevent access. LPA observed 12 children were present during this inspection in daycare classroom with licensee and her two staff having lunch in the dining room. Next LPA toured the bathroom in the hallway and observed child proof safety latches on the cabinets under the sink. The restroom that children use was observed to be safe and sanitary. Next LPA toured the off-limit kitchen and observed child proof safety latched on the cabinet where licensee stores sharp items and cleaning compounds.REPORT CONTINUES ON NEXT PAGE 1 of 4 |
