Licensing Program Analyst (LPA) Warren Birks conducted a Case Management Incident inspection. This inspection is regarding an incident that took place on November 9, 2023. LPA met with Licensee Brisa Ocean who provided information and assistance during the inspection.

Licensee guided LPA to the outdoor area to observe where child #1 was injured. LPA observed the area to appear to be in normal condition (orderly and free of uncommon hazards). LPA also observed the high chair that child #1 jumped from appear to be in adequate condition. Licensee indicated she was approximately one to two feet away assisting child #2 when child l#1 was injured. Child #1 stood up in the high chair and leaped falling to the ground sustaining an abrasion on the face and minor head injury. Licensee and Staff #2 indicated that the Licensee was in vicinity when the injury took place. Note: two children who witnessed the injury were not qualified for interview. LPA observed child #1 at the facility playing with no restrictions.

Post injury Staff #1 conducted first aid, called 911 and parent. Licensee and staff #1 indicated child #1 was taken to the hospital for medical treatment and returned to the childcare facility the same day with parent. Parent indicated child #1 was fine with no restrictions. Child #1 also was at the facility the next business day with no restrictions. Staff followed the required protocol in terms of first aid and "reporting requirements" as the 11/09/2023 incident was reported to Child Care Licensing within 24 hours (or the next business day). In addition, the Licensee provided a written incident report within the seven day requirement. The Information provided matches the incident report. Based on interviews and observations there were no violations of Title 22 requirements at this time.

The Notice of Site Visit (LIC 9213)must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with Licensee Brisa Ocean.
