Licensing Program Analyst (LPA) Lilli Babcock conducted an unannounced Case Management inspection due to an incident that occurred on 8/20/24 and was reported to the Department on 8/20/24. A COVID risk assessment was conducted. LPA met with Director, Christina Gudino to whom the reason for the visit was explained. Director guided LPA on a tour of the facility. Census was taken. There were 6 staff present caring for 44 children. The facility was observed to be operating within the license capacity limitations.

On August 20, 2024, an unusual incident report was submitted to the Department regarding an incident that involved a child who sustained an injury that required medical attention. The facility reported this incident to the Department within the required 24 hours. Based on information obtained from staff interviews and declarations, LPA Babcock determined that on 8/20/24 at approximately 12:15 pm, Child #1 (C1) was dancing to a music and movement song before nap time. C1 spun their body around, tripped on their feet, and fell hitting their forehead above the left eyebrow on the windowsill inside the classroom. C1 sustained a cut approximately 1/2 an inch long. First aid in the form of pressure applied to the wound was administered, and parents were notified of the incident. During the visit LPA attempted to interview C1, however C1 declined to talk with LPA. At 10:10 a.m. during this visit, LPA inspected the classroom and did not observe any hazards around the area where incident took place. LPA observed the facility has added a silicone padding to the edge of the windowsill where the incident occurred. Per Director the padding was added a few days after the

incident took place. Staff #1 stated the carpet area has been moved further away from the windowsill since the incident occurred, as another precaution. Per Director, there are no cameras in the classrooms, therefore no video footage was available for review.

Based on declarations and interviews from staff, there was adequate supervision (2 staff and 16 children) at the time of the incident. C1 was taken for medical treatment and per Director, medical staff glued the wound closed. C1 has returned to day care.

Based on information obtained during this investigation, no additional follow up is necessary regarding the incident reported. The facility followed all proper procedures; staff administered first aid, the facility was in ratio at the time of the incident, child’s parents were notified, incident report was sent in properly and timely, and all medical needs were met.

No deficiencies are being cited at this time.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
