25 | Licensing Program Analyst (LPA) Saul Valenzuela conducted an unannounced Case Management inspection due to three (3) incidents that were reported to the Department on 12/17/2024, 1/8/2025, and 1/17/2025. LPA met with Site Supervisor, Liliana Rubio who guided LPA on a tour of the facility. Census was taken.
On December 17th, 2025, an incident was self-reported to the Department via Email by the facility who reported that a child sustained an injury requiring medical treatment.
On January 8th, 2025, an incident was self-reported to the Department via Email by the facility who reported that a child sustained an injury requiring medical treatment.
On January 17th, 2025, an incident was self-reported to the Department via Email by the facility who reported that a child sustained an injury requiring medical treatment.
All reports were reported within the required 24 hours. The purpose of the inspection was to obtain additional information regarding the incidents reported to the Department.
During the inspection, LPA Valenzuela conducted interviews with staff. Staff disclosed that any child sustaining a head injury requires a doctor's note clearing the child to return to school. LPA reviewed Doctor's notes from children that sustained a head injury and were cleared to return to school with no restriction the following day of their incident.
