25 | ANNUAL INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced annual inspection to the above facility on 07/15/2021. LPA arrived at the facility at 1:40 PM and met with Lusine Sahakyants, Licensee who guided analyst on a tour of the facility. Also present during this inspection, were Mayranush Alaverdyan and Gayane Petosyan, Licensee’s Assistants. Per licensee, facility hours are Monday to Friday from 8:30 a.m. to 10:00 p.m.. Licensee is affiliated with Pacific Clinic Head Start Program.
This is a one story home located on the first level. The home consists of 3 bedrooms, 2 restrooms, living room, family room, kitchen, laundry room, dining area, garage, front yard, and backyard (fenced). Per licensee the children use the family room (daycare classroom), one bathroom adjacent to the family room, laundry room, and backyard fenced. Per licensee areas off limits to children and parents include: 3 bedrooms, 1 bathroom, living room, dining room, kitchen, front yard and garage. LPA did not observe stairs in the home. Per licensee, parents enter the facility through the side gate which leads to the backyard and daycare classroom. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 1:50 p.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the family room (daycare classroom) and observed 8 children napping with licensee's two assistants. LPA observed children toys, tables, and chairs in the family room (daycare classroom). LPA observed all electrical outlets were covered in daycare area. There is a wall heather in the family room (daycare area). Per licensee the wall heather is disconnected and inaccessible to the children. Next LPA toured the daycare bathroom adjacent to the family room. The restroom that children use was observed to be safe and sanitary. Next LPA toured the laundry room which is located in the hallway of family room and daycare bathroom and did not observe any hazards. LPA observed the kitchen door which leads to the rest of the home (off-limit areas) was closed. REPORT CONTINUES ON NEXT PAGE 1 of 4 |
