Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced case management inspection on 03/29/2022, due to an incident that occurred on Tuesday on 03/15/2022. LPA arrived at the facility at 1:20 PM and met with Lusine Sahakyants, Licensee who guided analyst on a tour of the facility. Also present during this inspection, were Gayane Petrosyan and Mayranush Alaverdyan, Licensee’s Assistants.

Alleged Incident took place on 03/15/2022. Incident was reported via telephone same working day. Original LIC 624B Unusual Incident/Injury Report form was received by the Department within 7 days. The written incident report was received by email on Tuesday 03/15/2022. The facility reported the incident within the required 24 hour time frame.

Licensee contacted LPA Eivazian on 03/15/2022 that Kid Care food program brought to her attention that she had 14 children enrolled in her daycare that all 14 children are preschoolers. Per licensee, she enrolled child#1 on 03/08/2022 and child #2 on 02/15/2022. Per licensee, both child#1 and child#2 were part timers and never 14 children were present at one time. Per licensee child#3 and child#4 from current enrolled children are part timers as well. Per licensee, she disenrolled both children as of 03/15/2022.
Per licensee, she completed food program claim list by mistake and she selected breakfast, lunch and dinner for child#1 and child#2 even though they were not present in the facility for whole day.

Ratio and staffing were in accordance of Title 22 Code of Regulations during this inspection.
LPA issued a Technical Violation in accordance with California Code of Regulations Title 22, Staffing Ratio and Capacity-- 102416.5 (a). The capacity specified on the license shall be the maximum number of children for whom care may be provided at any one time.
The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the Licensee, Lusine Sahakyants at 3:00 p.m..
