At 1:30pm, Licensing Program Analysts (LPA)s Roxana Lopez and Stephanie Li conducted an unannounced required 3 year inspection to the above facility. A risk assessment was conducted upon entry. LPA met with Yan Hong Yan Licensee who guided analysts on a tour of the facility. Per Licensee, there are currently 11 children enrolled. A current children’s roster was available for review. There were 9 children present upon arrival- 2 being infants.

Upon arrival, LPA’s observed that adult living in the home, Joshua Pulanco is not fingerprinted and cleared. During visit, Licensee had Joshua leave to get fingerprinted.This poses an immediate risks to health and safety of children in care, a civil penalty was assess on this date.

The licensee states that 7 adults and 0 children currently live in the home. Per Licensee, they currently have 1 assistant. Licensee states that there are no firearms stored in the home. This is a two story home which consists of 5 bedrooms, 6 restrooms, living room, family room, kitchen, garage and backyard (fenced). The children have access to day care room and the restroom located on the first floor near the living room and backyard fenced. There is no access to the front of the house from the day care area.

Per licensee, areas off limits to children and parents include entire second floor, 5 bedrooms, 5 bathrooms, kitchen, living room, and garage. The LPA toured all areas used by children during this visit. LPA’s observed a baby gate at the bottom making second floor inaccessible. LPA’s observed gate restricting access to living room and kitchen area making it inaccessible.

All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. Page 1 of 4

There is telephone service via a landline and a cellphone that stays at the facility during operation hours. There is ventilation and heating (central).

The following was observed and reviewed during this inspection. Detergents, cleaning compounds, and medications were observed to be inaccessible to children. Per licensee, there are no poisons are kept in the home. The restroom that children use was observed to be safe and sanitary.

At 2:00 pm LPA’s observed the valve on the required 2A 10BC fire extinguisher indicates last serviced on 10/22/2022 and not fully charged, as indicated on service tag. Smoke and carbon monoxide detectors were tested and are operable. All homes shall conduct fire and disaster drills at least once every six months, and document the date and time of each drill. Last drill documented was conducted on 11/5/2023.

Emergency Disaster Plan, Parent’s Rights Poster and the Facility License were observed to be posted on the parent board in living room.

The home is observed to be clean and orderly. Where children less than five years old are in care, stairs are fenced or barricaded. There is a child safety gate which keeps stairs inaccessible to children. There are toys available for children. Appropriate sleeping arrangements and play yards were observed.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA’s did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs, Trampolines and/or any other item that fall into these categories are not permitted in a family child care facility.

Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home.

Currently, children are using the back yard for outdoor play time. The outdoor play area was observed to be fenced. LPA’s observed that the outdoor yard has toys and other materials for children to play with. LPA’s did not observe any objects that can pose a danger to children on the outdoor yard. There are pets on the premises. There is one small dog in the home.

The licensee is observed to be operating within the license capacity limitations.

At 2:00 pm, children’s records were reviewed, including emergency information and were observed to be complete.

The licensee has completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 5/2025. There are first aid supplies available.

LPA observed that the Licensee and assistant do have proof of immunization record and the Mandated Reporter AB 1207 compliant Child Care Training Certificate on file.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Megan’s Law - Family Child Care Homes During the exit interview, the Licensee Yan Hong Yan, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809D (deficiencies page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.



LPA’s informed licensee Yan Hong Yan that this report dated 12/04/2023 documents and 1 Type A citation which shall be posted for 30 consecutive days as there is an immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA's Roxana Lopez and Stephanie Li informed the licensee Yan Hong Yan to provide a copy of this licensing report dated 12/04/2023 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days.
