25 | On 11/6/25, at 8:30AM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an Unannounced Annual/Random inspection to the above facility. Upon arrival, LPA met with Staff / Arusyak Mkrtchyan and was joined by the Licensee / Ani Simonyan shortly after, who assisted wit the visit. LPA disclosed the purpose of the visit. Entrance Checklist – Family Child Care Homes was provided to the Licensee. Licensee guided the LPA on a tour of the facility. Adults in the home were discussed and all have criminal record clearance. Also present were, Licensee’s Assistants / Arusyak Mkrtchyan and Monika Israylyan. There were 11 day care children present during today’s inspection. Licensee states that there are currently 11 children enrolled. The children's roster was reviewed and is current. Per licensee, the facility’s hours of operation are Monday - Friday, from 9AM - 10PM. Emergency Disaster Plan, License, and Parents’ Rights were posted at the time of inspection. Disaster drill log was also available during today’s inspection and the last disaster drill was conducted on 6/6/25.
This is a single story home which consists of 3 bedrooms, 2 bathrooms, play room/classroom, sleeping room/classroom, kitchen, outdoor office/storage room, backyard/parking lot, side yard/play area, and front yard. According to the Licensee, parents drop off and pick up their children at the main entrance, which leads to the play room/classroom. Areas that are accessible to children are as follows: Play room/classroom, sleeping room/classroom, kitchen, bathroom 1, and side yard/.play area. The home does not have a fireplace and washer/dryer. The side yard/play area has artificial grass with age appropriate toys. Per Licensee, off-limits areas are as follows: Bedrooms 1 -3, bathroom 2, outdoor office/storage room, backyard/parking lot, and front yard. Bedroom 3 is currently being used by the Licensee as a personal living room, rather than as a bedroom. The off-limits rooms are made inaccessible by child proof door knobs/locks.
