ANNUAL/RANDOM INSPECTION CONDUCTED IN ARMENIAN
Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced Annual/Random inspection to the above facility on 7/30/25. LPA arrived at the facility at 8:05 AM and met with Astkhik Davoian, Licensee who guided analyst on a tour of the facility. During this inspection Hripsime Galstyan, licensee's assistant arrived to the facility. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensee. Per licensee, facility hours are Monday to Friday from 7:30 a.m. to 6:30 p.m.. Per licensee she provides care to children 0-12 years old.

This is a one story home located on the first level. The home consists of 3 bedrooms, 1 bathroom, kitchen, living room, front yard, and backyard (fenced). Per licensee, parents enter the home through the main entrance which leads to the living room and side gate which leads to the backyard.

There is a detached ADU in the backyard with separated address, 1304 E. Harvard Street Unit A, Glendale, CA 91205.

There is a Family Child Care Home provider in the ADU at 1304 E. Harvard Street Unit A, Glendale, CA 91205 with facility number 198021238. Licensee in the ADU home, Elen Shakhgeldyan is licensee’s niece. Per licensee, back yard is used on a staggered schedule with Shakhgeldyan Family Child Care with facility number 198021238 for outdoor playtime on a staggered schedule. Shakhgeldyan FCC uses the outdoor from 11:15 AM to 12PM and 4:30PM to 5:15PM, and licensee, Astkhik Davoyan uses from 10:30 AM to 11AM and 5:45 PM to 6PM. Per licensee, there are times that due to weather staggered outdoor use will be vary but Davoian FCC children never commingle with Shakhgeldyan FCC children.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The children use the living room, 1 bathroom in the hallway, two bedrooms, kitchen, and backyard (fenced). Per licensee, areas off limits to children and parents include: 1 bedroom, and front yard. LPA did not observe stairs in the home during this inspection. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.

All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 8:15 a.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room, LPA observed children toys, tables, chairs, a TV and a piano in the living room. During this inspection at 9:00 AM 13 children were present in living room, one being school age child with licensee and her assistant. There is a fireplace in the living room which has been blocked off to prevent access. A child safety gate was installed to the hallway of bedrooms and kitchen entrance from living room. Next LPA toured the first daycare bedroom adjacent to the living room and observed children chairs and a table in the bedroom and it was set up as a classroom. Next LPA toured the second daycare bedroom adjacent to the kitchen and observed two pack n play and 5 beds in the bedroom. Next LPA toured the one bathroom in the hallway and observed child proof safety latches on the cabinet under the sink. LPA checked the cabinet under the sink in the bathroom and observed a "Raid, Ant and Roach" spray in the under the sink cabinet. Next LPA toured the kitchen and observed a children table and chairs, two high chairs, one adult table and four chairs in the kitchen. Per licensee, children either eat outside or in the kitchen. LPA observed child proof safety latches on the cabinets where licensee stores sharp items and cleaning compounds. LPA checked the under the sink cabinet and observed "Raid, Ant and Roach" spray in the cabinet. There are first aid supplies available on the premises in the kitchen. LPA observed the required 2A 10BC fire extinguishers were anchored to the wall by the entrance door and in the kitchen. There were serviced on 07/05/25 as read by LPA from the service tag. Licensee tested the carbon monoxide and smoke detectors in the hallway of bedrooms. Both sounded off the alarms and heard by LPA to be functional. Detergents, cleaning compounds, medications, and other items which could pose a danger to children were observed to be inaccessible to children. LPA informed licensee, poisons (insect and bug spray) storage areas for poisons must be locked with a key or combination lock.

Next LPA toured the backyard fenced. Currently, children are using the back yard (fenced) for outdoor play time. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA did not observe any objects that can pose a danger to children on the outdoor yard. The licensee states that supervision is always provided. LPA observed the off-limit back of backyard and off-limit detached ADU in he backyard is separated by a gate to make it inaccessible to the children.

The licensee has completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 02/27. Licensee, completed required mandated reporter training on 06/23/25. LPA advised licensee that if a child shows signs of illness he/she/they shall be separated from other children.

At 9:30.m., facility roster was reviewed and was current. Per licensee currently there are 14 children enrolled. LPA reviewed 6 children's records, including emergency information from 14 enrolled children, and all 6 reviewed files were completed. The licensee and her one assistant have proof of immunization against influenza, pertussis, and measles. All homes shall conduct fire and disaster drills at least once every six months, and document the date and time of each drill. Last drill documented was conducted on 06/02/25. Emergency Disaster Plan, Parent’s Rights Poster and the Facility License were observed to be posted at the wall by the entrance in the living room.

LPA did not observe the following items during the inspection: Infant Walkers, Johnny Jumpers, Saucer Chairs, Trampolines and/or any other item that fall into these categories are not permitted in a family child care facility. Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home. There is telephone service via a landline that is used and stays at the facility during operation hours. Per licensee she carries liability insurance in accordance with standard established by Family Child Care statue.

SAFE SLEEP: LPA discussed the safe sleep regulations with licensee, and discussed the Child Care Licensing Safe Sleep webpage at http://www.cdss.ca.gov/inforescources/child-care-licensing/public-information-and-resoucrces/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at http://www/cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. INFANT CARE: Licensee states that she is providing care to infants and currently has two infants, 20 and 22 months old enrolled in her daycare. Licensee states that infants sleep in the one daycare bedroom and living room where they are constantly supervised. Appropriate sleeping arrangements and cribs were observed. (one crib for each infant in care was observed). Cribs or play yard did not hinder the entrance or exit from the sleeping space mattresses shall be firm and covered with a fitted sheet that overlaps the underside so it cannot be dislodged. Cribs and play yards were observed to be free of loose articles and objects. No objects were observed to be hanging above or attached to the side of the crib. LPA did not observe any infants swaddled while in care. LPA advised the Licensee that infants shall be placed on their backs for sleeping and shall be supervised. Infants shall be checked on every 15 minutes and the time of each 15-minute check shall be documented with child’s name and date. Per licensee she did not complete infant nap chart for two enrolled infants.

Medication: Incidental Medical Services (IMS) policy was discussed. The licensee states that she will provide IMS. Per licensee, there are no children enrolled that require IMS at this time. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA Eivazian reviewed and discussed with the licensee the following Title 22 Regulations to ensure licensee maintain compliance and meet the Title 22 regulations at all times.

Definitions--102352 -- "Family Day Care" or "Family Child Care" means regularly provided care, protection and supervision of children, in the care giver's own home, for periods of less than 24 hours per day, while the parents or authorized representatives are away. The term "Family Child Care" supersedes the term "Family Day Care" as used in previous regulations and "Home" means the licensee's residence as defined by Government Code Section 244.

Operation of a Family Child Care Home--102417 (a)-- The licensee shall be present in the home and shall ensure that children in care are supervised at all times. When circumstances require the licensee to be temporarily absent from the home, the licensee shall arrange for a substitute adult to care for and supervise the children during his/her absence. Temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day.

Definitions--102352 (a) (3)--"Assistant Provider" means a person at least 14 years of age who is primarily involved in caring for children during the hours that the home provides care.

Reporting Requirements--102416.2 (a) (2)--Any change in household composition including adults moving in or out of the home and anyone living in the home who reaches his or her 18th birthday.



Inspection Authority of the Department—102391 (a)--Any duly authorized officer, employee, or agent of the Department shall, upon presentation of proper identification, enter and inspect any place providing personal care, supervision, and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation of, the regulations adopted by the Department governing family child care homes, and in accordance with Section 102396.

Staffing Ratio and Capacity--102416.5 (a)--The capacity specified on the license shall be the maximum number of children for whom care may be provided at any one time.

Per licensee, Astkhik Davoian facility Rosters and children's files for enrollment will be updated all the time.
- Licensee will be the main person to provide care to the children and interact with families.
- Licensee will interact with Department staff professionally and respectfully at all times. Licensee understands that inspections are conducted in person, unannounced, and Department staff will be granted access to the facility as part of the inspection process.
- Licensee will be forthright with the department all the times to ensure the health and safety of children in care.
- Licensee will keep her daycare children records at this address all the time.

The following deficiencies listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.
A notice of site visit was given and must remain posted for 30 days.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Licensee, Astkhik Davoian was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.



During the exit interview, the Licensee, Astkhik Davoian confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Based on tour of the facility, the licensee did not comply with the section cited above and LPA observed child proof safety latch on the cabient under the sink in the kitchen and bathroom but LPA observed "Raid, Anut and Roach " spray in the cabient which consider as poison and it was not locked which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on children file review, the licensee did not comply with the section cited above, per licensee, she did not complete infant nap chart for currently enrolled two infants which poses/posed a potential health, safety or personal rights risk to persons in care.
