25 | ANNUAL INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analysts (LPA) Anomeh Eivazian and Steven Tung conducted an unannounced annual inspection to the above facility on 08/12/2021. LPAs arrived at the facility at 1:56 PM and met with Olga Ghazaryan, Licensee who guided analyst on a tour of the facility. Also present during this inspection, were Margarite Ghazaryan and Silvana Isaian, Licensee’s Assistants. Per licensee, facility hours are 24 hours a day, for periods of less than 24 hours a day care.
This is a single family two story house. The home consists of 4 bedrooms, 2 and 1/2 restrooms, living room, dining room, kitchen, front yard, and backyard (fenced). There is a fireplace in the living room which has been blocked off to prevent access to children and per licensee it is disconnected.
Per licensee the children use the living room , dining room, two bedrooms in the first floor, half bathroom and backyard (fenced). Per licensee areas off limits to children and parents include: 2 bedrooms and one bathroom in the second floor, kitchen, and front yard. There are stairs in the home which leads to the second floor. LPAs observed a gate was installed to the stairs which leads to the second floor. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 2:00 p.m., Licensee took this LPAs on a tour of the home. LPAs was toured through the interior starting with the living room and dining room (daycare classrooms), LPAs observed sofas, a coffee table, a TV, adult dining table, chairs, children's table, and chairs in the dining room and living room. LPAs observed all electrical outlets were covered in living room and dining room. Next LPAs toured the two bedrooms in the first floor and observed 7 children were sleeping in the bedrooms. Next LPA toured the half bathroom in the daycare bedroom and did not observe any hazards.REPORT CONTINUES ON NEXT PAGE 1 of 4 |
