REQUIRED- 3 YEAR INSPECTION CONDUCTED IN ARMENIAN
Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced required- 3 year inspection to the above facility on 09/03/24. LPA arrived at the facility at 8:40 AM and met with licensee's assistants, Liana Melkonyan and Alvina Aleksanyan. Upon LPA's arrival to the home, licensee was not present in the home. At 9:20 AM licensee arrived to the facility who stated she went to visit her mother. Licensee, Olga Ghazaryan guided analyst on a tour of the facility at 9:40 AM. A COVID 19 risk assessment was conducted prior to entering the facility. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensee. Per licensee, facility operation hours are Monday to Sunday, 8:00AM to 10:00 PM. Per licensee, she provides care to children 1-14 years old.

This is a single family two story house. The home consists of 4 bedrooms, 2 and 1/2 restrooms, living room, dining room, kitchen, front yard, and backyard (fenced). Per licensee, parents enter the home through the main entrance door which leads to the living room.

Per licensee the children use the living room , dining room, two bedrooms in the first floor, half bathroom in the daycare bedroom, kitchen and backyard (fenced). Per licensee areas off limits to children and parents include: 2 bedrooms and one bathroom in the second floor, one bathroom in the first floor, and front yard. There are stairs in the home which leads to the second floor. LPA observed a gate was installed to the stairs which leads to the second floor. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report. An updated Family Child Care Home Application LIC 279 will be submitted by 09/20/24.

All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 9:20 a.m., Licensee took this LPA on a tour of the home. LPA was toured

through the interior starting with the living room and dining room (daycare classrooms), LPA observed sofas, a coffee table, a TV, adult dining table, chairs, children's table, and chairs in the dining room and living room. LPA observed all electrical outlets were covered in living room and dining room. There is a fireplace in the living room which has been blocked off to prevent access. Next LPA toured the two bedrooms in the first floor and observed children toys, chairs, tables, beds in the two bedrooms. Next LPA toured the half bathroom in the daycare bedroom and did not observe any hazards. Next LPA toured the kitchen and observed a knives set was placed on the kitchen cabinet, knives in the drawer and cleaning compounds (clorox sprays) were accessible to children in the cabinet under the sink. Per licensee, children only walk through the kitchen to go to the backyard. Next LPA toured the off-limit second floor and observed family members personal belongings in the home. LPA observed the required 2A 10BC fire extinguisher was anchored to the wall in the living room and it was serviced on 09/03/24 as read by LPA from service tag. Licensee tested the carbon monoxide and smoke detectors in the daycare bedroom. Both sounded off the alarms and heard by LPA to be functional.

Next LPA toured the backyard fenced. Currently, children are using the back yard for outdoor play time. During this inspection LPA observed 10 children were playing outdoor with licensee's two assistants. The outdoor play area was observed to be fenced. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA did not observe any objects that can pose a danger to children on the outdoor yard. The licensee states that supervision is always provided.

Per licensee, she provides food for children in care. Licensee was advised that if food is brought from the children’s homes, all containers must be labeled with child’s name and properly stored or refrigerated. Per Licensee, Olga Ghazaryan at 10:20 a.m there are no pets, weapons, firearms, pools, spas, hot tubs, fish ponds, or similar bodies of water on the premises.

The licensee was observed to be operating within the licensed capacity and is not exceeding the required limitations during this inspection. During this inspection 10 children were present in the home with licensee and her two assistants. All adults present have obtained a criminal record clearance on Licensing Information System (LIS), but licensee's son, Arman Khachatryan who lives at this address over a year does not have criminal record clearance on Licensing Information System. Per licensee, currently she has two assistants. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The licensee and other personnel have completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 09/2024, licensee's assistant Liana Melkonyan Pediatric First Aid and CPR expires 06/2026. Licensee, Olga Ghazaryan completed required mandated reporter training on 12/19/21, but has not been updated every 2 years as it is required. There are first aid supplies available on the premises in a closet in the daycare bedroom. LPA advised licensee that if a child shows signs of illness he/she/they shall be separated from other children.

At 10:50 a.m., facility roster was reviewed and completed. Per licensee currently there are 11 children enrolled. LPA reviewed 5 children's records, including emergency information from 11 enrolled children, and all 5 reviewed files were completed. Per licensee, she provides transportation if it will be needed.


The licensee and her assistant, Liana Melkonyan have proof of immunization against influenza, pertussis, and measles. Licensee's assistant, Alvina Aleksanyan does not have proof of immunization against influenza, pertussis, and measles on file.

All homes shall conduct fire and disaster drills at least once every six months, and document the date and time of each drill. Last drill documented was conducted on 08/16/24.

Emergency Disaster Plan, Parent’s Rights Poster and the Facility License were observed to be posted at the wall in the daycare room. Smoking is prohibited in a licensed Family Child Care Home. Per Licensee, no one smokes in the home. There is telephone service via a cellphone that is used and the cellphone stays at the facility during operation hours. Per licensee she carries liability insurance or a bond in accordance with standard established by Family Child Care statue.

INFANT CARE: Licensee states she provides care to infants (12-24 months old). Per licensee, currently she has one infant 18 months old enrolled in her daycare. Licensee states that infants sleep in the two bedrooms where they are constantly supervised. LPA advised the Licensee that infants shall be placed on their backs for sleeping and shall be supervised. Infants shall be checked on every 15 minutes and the time of each 15-minute check shall be documented with child’s name and date. The LIC 9227 Individual Infant Sleeping Plan shall be completed for each infant up to 12 months of age. A copy of the LIC 9227 was provided to Licensee.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Medication: Incidental Medical Services (IMS) policy was discussed. The licensee states that she will provide IMS. Per licensee, there are no children enrolled that require IMS at this time. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. The following deficiencies listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

LPA Eivazian informed licensee, Olga Ghazaryan that this report dated 09/03/24 document(s) (2 Type A citation) Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Eivazian informed the licensee, Olga Ghazaryan to provide a copy of this licensing report dated 09/03/24 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
