25 | On 9/24/25, at 12:30PM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an Unannounced Annual Continuation inspection to the above facility. Upon arrival, LPA disclosed the purpose of the inspection and met with Licensee, Ofelya Sargsyan who guided the LPA on a tour of the facility. Adults in the home were discussed and all have criminal record clearance. Also present was Licensee’s Assistant / Alvina Aleksanyan. There were 7 day care children present during today’s inspection. Licensee states that there are currently 11 children enrolled. The children's roster was reviewed and is current. Per licensee, the facility’s hours of operation are Monday - Sunday, from 8AM - 10PM. Emergency Disaster Plan, License, and Parents’ Rights were posted at the time of inspection. Disaster drill log was also available during today’s inspection, the last disaster drill was conducted on 7/9/26.
This is 2 story home which consists of 4 bedrooms, 3 bathrooms, living room, dining room, front yard and back yard. There is a second home located on the same property (in the back), with a separate address: 509A E Windsor Rd, Glendale, CA 91205. Tenants of the 509A unit access their residence through the main driveway, which leads to Unit A, separated by an enclosed gate. According to the Licensee, parents drop off and pick up their children at the main entrance, which leads to the living room. Areas that are accessible to children are as follows: living room, dining room, bedrooms 1 and 2 (both located on the first floor), bathroom 1 (a half bathroom inside bedroom 1), kitchen, and the enclosed backyard. The living room has a fireplace which has been screened to prevent access by children. Per Licensee, the fireplace will not be used. The back yard has artificial grass with age appropriate toys. Per Licensee, off-limits areas are as follows: front yard, bathroom 2 (which is located on the first floor) and the entire second floor, which contains bedrooms 3 and 4, as well as bathroom 3. The off-limits rooms are made inaccessible by child proof door knobs/locks. Access to the second floor is prevented by a child safety gate positioned at the bottom of the stairs.
