25 | Licensing Program Analyst (LPA) Nolan Tcheng conducted an unannounced Case Management inspection for the purpose of addressing deficiencies at the facility. Upon arrival at 8:30am, LPA met with Director Rosalie Reyes, to whom the purpose of the inspection was explained. Tour was provided of the facility. There were children present during the time of the inspection.
Census was taken. There were 4 children with 2 staff member.
During an inspection on 09/28/2023, LPA observed that the infant room had moved from the original licensed space to the empty preschool room (part of #198018204). Review of the facility file and facility sketch corroborated the current room being used is licensed for preschool age children. On today's date, 4 children of infant enrollment were observed in the preschool licensed classroom. This is a violation of the conditions of the license for this facility. LPA observed that there was no sleeping space for infants in the preschool room that was physically separated from the activity space. The cribs were observed to be placed around the classroom along the walls surrounding the activity space. Upon additional review of the preschool licensed classroom being used, LPA observed that the changing table present in the classroom is not within arms reach of an eligible sink. LPA discussed with director the requirement for changing tables.
Director stated they are renovating the infant licensed classroom and moved the class to this empty preschool room until the renovations were completed. LPA observed that the Department has not been notified of the renovations being conducted in children activity space. LPA discussed with Director the requirement to report construction/renovations that take place on facility grounds.
During inspection, LPA instructed Director to contact families and have all children picked up. LPA observed all children picked up during inspection. Facility should remain closed and not have children in attendance until the approved licensed space has completed renovations and is safe for children in care to return.
