25 | CASE MANAGEMENT INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced case management inspection to the above facility on 07/30/2021 due to licensee requesting a capacity increase. LPA arrived at the facility at 11:55 AM and met with Svetlana Iskandaryan, Licensee who guided analyst on a tour of the facility. Also present during this inspection, was Davit Yeghiazaryan, Licensee’s Son. Per licensee, facility hours are Monday to Friday from 8:00 a.m. to 5:00 p.m. Fire inspection clearance for Large Family Child Care Home was granted on 07/16/2021.
This is an apartment complex located on the first level. The home consists of 3 bedrooms, 2 restrooms, living room (day care room), kitchen, front yard, backyard, and side yard (fenced). Per licensee children use the bathroom in the hallway, living room (day care room), kitchen, two bedrooms, and side yard (fenced). Per licensee parents enter the facility through the side gate which leads to the side yard and living room (day care room). Per licensee areas off limits to children and parents include: 1 bedroom at the end of the hallway, one bathroom in the master bedroom, front yard and back yard. LPA did not observe stairs in the home. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 12:10 p.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room (daycare classroom), LPA observed sofas, a TV, children's table, chairs, and toys in the living room. LPA observed all electrical outlets were covered in living room. Next LPA toured the kitchen and observed child proof safety latches on the cabinets where licensee stores sharp items and cleaning compounds. LPA observed a safety gate was installed between kitchen and living room.
Next LPA toured the master bedroom and observed a full size bed. Per licensee her school age children study in the master bedroom. Next LPA toured the off-limit bathroom in the master bedroom and did not observe any hazards. REPORT CONTINUES ON NEXT PAGE 1 of 4 |
