25 | Licensing Program Analyst (LPA) Stephanie Li conducted an unannounced Case Management inspection due to an incident that occurred on 11/7/24 and was reported to the Department on 11/8/24. A COVID risk assessment was conducted. LPA met with Assistant Director Carolyn Scott to whom the reason for the visit was explained. Director guided LPA on a tour of the facility. There were 55 children present and 8 staff on the 198021260 license, which the children who got injured are enrolled. The facility was observed to be operating within the license capacity limitations.
On 11/8/2024, an unusual incident report was made to the Department regarding an injury that occurred during a contracted cooking class. Two children got injured on their hands from cooking tools (butter knife and kid safety peeler) First aid was provided and parents were notified of injuries through the tadpoles app. Both children went to urgent care and returned within a day.
During todays visit, LPA interviews assistant director Carolyn Scott, observed the cooking area, and verified which facility preschool children were in the cooking class. Per assistant Director Carolyn, teaching staff walk children into waiver room and drop group off to contracted teacher and return to the upper preschool program. Facility has a file for contracted vendor as she was a previous employee. Vendor is fully qualified, has current CPR and mandated reporter certificate and is associated to both preschool facility license.
Based on information obtained during this investigation, follow up is necessary regarding the incident reported. No deficiencies are being cited at this time.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
