25 | REQUIRED- 1 YEAR INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced required 1- year inspection to the above facility on 09/22/23. LPA arrived at the facility at 8:20 AM and met with Emma Kostanyan, Licensee who guided analyst on a tour of the facility. A COVID 19 risk assessment was conducted prior to entering the facility. Also present during this inspection, was Gohar Sirakanyan, licensee's assistant and Vilik Kostanyan, Licensee’s husband. At 11:00 AM Nora Sergoyan, licensee's assistant arrived to the facility. Per licensee, Emma Kostanyan facility hours are Monday to Saturday from 7:00 a.m. to 10:00 p.m. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensee.
This is a two story home. The home consists of 3 bedrooms, 2 restrooms, living room, dining room, office
(daycare playroom), kitchen, detached converted garage, front yard and backyard (fenced). Per licensee the children use the living room, dining room, 1 bedroom (family room), office (daycare playroom), half bathroom, and backyard (fenced). LPA observed three locks on the main entrance door. Per licensee areas off limits to children and parents include: Kitchen, entire second floor which consists of two bedrooms, 1 bathroom, detached converted garage, and front yard. LPA observed a safety gate was installed to the stairs which leads to the second floor to make the second floor inaccessible to the children. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report. An updated facility sketch was submitted during this inspection. Per licensee, parents enter the home through the main entrance door which leads to the living room.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 8:50 a.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room (day care area), LPA observed a piano, children tables and chairs in the living room. LPA observed the fireplace was screened in the living room to prevent access. Next LPA toured the dining room and observed children tables and chairs in the dining room. Next LPA toured the bedroom (family room) and observed a TV and sofas in the family room. Next LPA toured the off-limit kitchen and observed a child safety gate was installed to the kitchen to make the kitchen inaccessible to the children.REPORT CONTINUES ON NEXT PAGE 1 of 5 |
