32 | Currently children use the front yard (fenced) for outdoor play with adequate shade and age-appropriate play equipment for children in care. LPA observed a small trampoline with some tears on the skirt and base. Per licensee, children use the trampoline one by one or in two’s and are supervised at all times. LPA advised the licensee to not allow the use of trampolines as recommended on our website under SHINE Element 6: Physical Activity. LPA provided a copy of the SHINE Element 6: Physical Activity. Trampoline should be replaced and follow manufactures guidelines. LPA also advised licensee to check with the American Standard Testing Materials for Playground Guidelines and/or to use the manufactures recommendations for the playground equipment in efforts to be more in line with safety guidelines. Licensee understands that children should be supervised at all times.
Children’s records were reviewed for (LIC) 282- Affidavit Regarding Liability Insurance, Immunization's Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, and LIC 995A Notification of Parents’ Rights, LIC 9227- Infant sleep form (0-12 months, and documentation of 15-minute Infant Sleep Check (0-24 months). LPA observed Child 1 (C1), C2, C4, and C6 are missing LIC 995A. LPA also observed licensee has been documenting infant safe sleep every 15-minutes; however, licensee is not consistent. Safe sleep log for the month of May for C2 documents only 05/06, 05/09, 05/13, 05/14, 05/17, 05/20 and for C6 05/06, 05/07, 05/13, 05/14, and 05/20. According to the licensee, C2 and C6 have been absent and therefore she did not document that on the safe sleep log.
