CASE-MANAGEMENT INSPECTION CONDUCTED IN ARMENIAN

Licensing Program Analyst (LPA) Anomeh Eivazian conducted a case-management unannounced inspection to the above facility on 05/22/24 due to licensee is requesting capacity decrease. LPA arrived at the facility at 10:10 AM and met with Ashkhen Bakhshyan, licensee's daughter/assistant who guided analyst on a tour of the facility. Also present during this inspection, were Svetik Melkumyan, licensee, Arusvak Bakhshyan, licensee's daughter/assistant and Karine Arakelyan, Licensee’s Assistant.

During this inspection seven children were present in the facility with licensee and her three assistants.

During this inspection licensee decided to withdraw her decrease of capacity application. A written statement was obtained from licensee to request to withdraw her decrease of capacity at this time.

At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today from the areas that were inspected today.

The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.



Exit interview conducted and report was reviewed with the Licensee's daughter, Ashkhen Bakhshyan and Svetik Melkumyan, licensee.
