25 | CASE MANAGEMENT INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced case management inspection to the above facility on 09/27/2021 due to licensee requesting capacity increase. LPA arrived at the facility at 12:30 PM and met with Lusine Sanoyan, Licensee who guided analyst on a tour of the facility. Also present during this inspection, were Artavazd Karamyan, Licensee’s husband and Anzhela Poghosyan, Licensee's assistant. Per licensee, facility hours are Monday to Friday from 7:00 a.m. to 10:00 p.m. Fire clearance for Large Family Child Care Home was granted on 09/24/2021.
All areas identified on the facility sketch were inspected. This is a one story home that is located on the first level. The home consists of living room, dining room, kitchen, 3 bedrooms, 2 bathrooms, front yard, backyard (fenced), converted garage that consists of laundry room and storage. There is a fireplace in the living room which has been blocked off to prevent access to children. Per licensee the children use the Living room, dining room, kitchen, master bedroom, bathroom in the master bedroom, bedroom#2, and backyard (fenced). Per licensee, parents enter the facility through the side gate which leads to the backyard and master door. Per licensee areas off limits to children and parents include: Bedroom #3, 1 bathroom, front yard, and converted garage which consists of laundry room and storage. An updated facility sketch was submitted during this inspections. LPA did not observe any satires in the home. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 12:45 p.m., Licensee took this LPA on a tour of the home. LPA was toured through the interior starting with the living room. LPA observed sofas, a coffee table, and TV in the living room. There is a fireplace in the living room which has been blocked off to prevent access. Next LPA toured the dining room and LPA observed dining table, chairs, children table, chairs and toys in the dining room.
