------------------------ Amended Report ------------------------------------------------------------
Licensing Program Analysts (LPA’s) Roxana Lopez and Lilli Babcock conducted an unannounced poc (plan of correction) inspection to insured that the 2 Type A's and 1 Type B deficiencies cited on 3/21/2022 have been cleared. LPA’s met with Gabriela Gallardo, licensee who guided analysts on a tour of the facility. The following was observed:

Licensee is not in compliance with the limitations of the license. A small family day care home may provide care for more than six and up to eight children- At least one child is enrolled in and attending kindergarten or elementary school and a second child is at least six years of age. LPA's observed 7 children all under the age of . This poses an immediate risk for the health and safety of children in care.

Assistant Silvana Gallardo is fingerprint cleared and associated to the facility. Licensee, serviced the expired extinguisher on 03/22/22 and bought an additional extinguisher on 3/25/22. LPA's did observe the Notice of Site Visit Posted visible for parents and the 9224 Acknowledgement forms in the children's file. LPA’s issued POC clearance letter during the visit for fingerprints and fire extinguisher.

Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety. Due to technical difficulties page 809D was unable to be created. LPA advised Licensee, to provide parents with the 9224 Acknowledgment form.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Licensee, Gabriela Gallardo
