25 | On May 3, 2024, at 1:00 pm Licensing Program Analyst (LPA) Carolyn Tuba conducted a case management for a repeat violation. Covid Risk assessment was conducted. LPA met with Licensee, Maria Flores. During the visit Licensee was called away and staff member, Arely Gomez took over the visit. LPA obtained the census of 11 children with 3 staff.
LPA conducted interviews with Parent #1 (P1), #2 (P2) #3 (P3) and Staff #1 (S1) & #2 (S2). During a previous inspection on 3/18/2024 a Type-A citation was issued in accordance and identified with as California Code of Regulations Title 22, 101229(a)(1). During today’s visit the same citation is being issued due to interviews conducted by P1, P2, and P3 who have observed at least 2 children who are of infant/toddler age being co-mingled with approximately 5 preschoolers during morning drop-off during the last part of the month of April 2024. S1 and S2 disclosed that they have had 1 infant with 2 to 3 preschoolers, during the time of 7:20 am – 7:30 am in the last week of April 2024.
The following deficiency listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiency that are being cited need to be cleared to protect the children’s health & safety. A civil penalty of an immediate $250 is being assessed for a repeat violation.
One (1) Type A - A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Licensee/Director was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Staff, Arely Gomez.
