32 | In regards to allegation Licensee is documenting hours for services not provided, Licensee does not maintain staff personnel records and Staff do not ensure that food is provided in the quantity necessary to meet the needs of the children in care. LPA conducted interviews with staff # 1- 7. Director disclosed that the agency they work with pays a set amount per child and that if children are out they write absent on the day that they are out and do not request parent signatures. Additionally, Director stated that they have files and documentation for all of the employees, including volunteers. Staff #1-7 disclosed that facility provides breakfast, lunch and snack. Menu is provided to parents in advanced and that if an item has to be replaced that parents are informed during pick up or sometimes its posted in class dojo.
LPA conducted interviews with children. Children’s statements corroborated that they like attending the facility and did not share any concern regarding the meals provided. LPA conducted interviews with parents. Statements corroborate that they do not have any concerns with the facility, sign in issues or the food being provided to the children. LPA did observed files for all staff in the facility including volunteers and observed agency signing sheets during the visits- were absences were documented.
This agency has investigated the complaint alleging Licensee is documenting hours for services not provided, Licensee does not maintain staff personnel records and Staff do not ensure that food is provided in the quantity necessary to meet the needs of the children in care.
Based upon the evidence as presented above, the allegation has been determined to be Unsubstantiated. A finding of Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove alleged violation(s) did or did not occur, therefore at this time the above allegation is unsubstantiated. No citations cited for this complaint investigation.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with Licensee Pubudu Karalliyadde. |
