On 08/25/2026 Licensing Program Analyst (LPA) Mariah Aguirre conducted an unannounced Annual/Random inspection at the above facility. During the exit interview, after final printing the report, staff member informed LPA MAguirre that the licensee, Sarah Ann Loeb does not live in the home, and uses the home as a daycare space.

Upon arrival, LPA was greeted by Lead Teacher Cora Chandler who guided LPA on a tour of the facility. At the time of the inspection, there were 6 napping children in care. Per Lead Teacher there are currently 12 children enrolled. Hours of operation are Monday through Friday 9:00AM to 4:00PM. Per Lead teacher, no adults live in the home.

Upon arrival at the facility, teaching staff informed LPA that the licensee had left the state for an emergency and would return on 08/30/2026. LPAs informed the Licensee that in the event an absence takes place, an Unusual Incident Report Form, LIC624 must be submitted to the department within the required time frame specified on the regulation. As a result, one Technical Violation was issued.

This is a single-story house with 2 bedrooms and 1 bathroom. Area's accessible to children in care is the living room used as a play space, den area, kitchen, one bathroom, and backyard, Per teaching staff, none of the bedrooms are accessible to children in care. Per Licensee, all inaccessible areas of the home are made inaccessible by locked doors, baby gates, and child safety locks. LPA did not observe any pets at the time of inspection; per teaching staff there are no pets in the home. At the time of inspection, LPA MAguirre observed the converted garage to be used as an art studio for children in care. LPA observed art equipment, and tables and chairs for the children. Due to new Fire Department regulations, use of garage as a daycare activity space is no longer permitted per CBC Chapter 4.406 stating: a. Garages are not considered habitable space or part of the “dwelling unit” where day care occurs unless converted. & b. Required exits may not pass through a garage. Due to the new regulation, LPA MAguirre and LPA AEivazian spoke with the Licensee Sarah Leob who was informed to discontinue use of the garage as an activity space immediately. Licensee stated she understands the new regulation and will discontinue garage use effective tomorrow, 08/26/2026.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPA observed napping equipment at the time of inspection. Napping equipment provided is mats. Per teaching staff, parents provide bedding for children in care. The bedding is sent home every Friday to be washed and is brought back to the facility every Monday.

The home is comfortable and well ventilated. Per teaching staff, meals are not provided to the children. The facility provides AM and PM snacks to children in care. LPA observed dual functioning smoke and carbon monoxide detectors throughout the home. LPA observed a working telephone and a fire extinguisher last serviced 04/28/2026.

There are several age-appropriate toys and a first aid kit on the premises. Licensees First Aid and CPR expired on 08/2027. LPA observed all cleaning products to be kept in a kitchen cabinet underneath the sink inaccessible to children in care. Per teaching staff, there are no poisons or firearms in the home.

LPA reviewed children’s files, files were found to be complete. LPA reviewed Licensee’s file, file was found to be complete. Children and Licensee’s names were documented on an LIC811. LPA observed a current facility roster at the time of inspection, and all required postings were observed on the licensee’s parent board. LPA reviewed staff files, files were found to be incomplete.

The licensee has completed the online mandated reporter training at www.mandatedreporterca.com, Licensee’s Mandated Reporter training expires on 04/17/2028. All childcare employees must complete mandated reporter training within 30 days of hire and renew training every two years. During file review, Staff #1 had a Mandated Reporter training certificate that expired on 02/11/2026. As a result, one Type B citation as issued.



Incidental Medical Services (IMS) policy was discussed. No children currently enrolled require IMS. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Teaching staff was advised of the requirement to report unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC624B.

The licensee was informed of the responsibility to report suspected Child Abuse by calling the Child Abuse Hot-line at 1-800-540-4000. Also call the CCLD office and follow up with a written Unusual Incident/Injury Report (LIC 624B).

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Per the Licensee, fire and disaster drills are conducted every six months. The last drill documented and conducted was May 1st 2026. LPA MAguirre observed a current drill log at the time of inspection.

Licensee has the required documents posted in the FCCH; Facility License (LIC 203), Emergency Disaster Plan (LIC610a), Notification of Parents' Rights Poster (PUB 394).

The following was discussed with the licensee:


Capacity requirements, Roster requirements, Posting requirements, Documentation requirements for disaster drills (fire and earthquake). Mandatory Forms for the children’s files and provider’s files, and Safe Sleep Regulation. The role and responsibilities of being a mandated reporter were reviewed. Licensee was reminded that supervision is always required to children in care.

Licensee was made aware that it is their responsibility to know the regulations as well as anyone who assists in providing care. Licensee was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Licensing must always have the facility’s phone number; if the phone number is changed, licensing must be notified.

Regulation prohibits the smoking of tobacco in a private residence that is licensed as a family childcare home and in those areas of the family day care home where children are present (24/7 ban). State law prohibits baby walkers, bouncy seats, exersaucers and any other items that fall into that category.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Lead Teacher Cora Chandler confirmed that there are no Registered Sex Offenders living in the home and LPA completed the RSO profile in FAS.
