LPA Seung Lee conducted an unannounced case management inspection. Upon arrival LPA Lee met with Director Joshua Goldman.

The purpose of the inspection was to obtain additional information on an incident report submitted by the facility to the child care regional office on 05/03/2023. A prior inspection was conducted on 05/04/2023 by LPA Lee to obtain information. During the course of this inspection, LPA Lee conducted interviews and made observations in regards to the incident that occurred on 05/03/2023 at the facility.

On 05/03/2023 at around 12:25PM two children in care were brought back to the facility by a neighbor who lives a few houses away from the facility. During an interview, the neighbor stated that Child#1 and Child#2 were both walking away from the facility main entrance towards the end of the alley in the direction of the Cristo Rey Church on Perlita Ave. The neighbor approached the two children and asked where they were going. One of the two children indicated they were from the facility, and the neighbor got out of their vehicle and escorted the two children by foot to the facility.

The neighbor stated that the time period from first discovering the two children to escorting them back was about 3 minutes. The Director stated in an interview that both parents of Child#1 and Child#2 were notified of this incident on 05/03/2023 by phone. Child#1 and Child#2 no longer attend this facility due to graduating. Based on the interview conducted with the neighbor Child#1 and Child#2 were able exit the main entrance and walk past least 3 homes on La Clede ave before being discovered by the neighbor. The two children were walking in the alley and not on La Clede ave. The facility was not aware of the two children wandering away until the neighbor brought them back.

The following deficiencies listed on the attached LIC 809D are being cited in accordance with California Code of Regulations Title 22. A copy of the LIC 9224 was provided to the facility during this inspection. The Director understands that a signature on the copies of this form is to be filed for every child currently attending and any new children enrolled for a period of 1 year starting from this date.

The notice of site inspection along with all pages of this report must remain posted for 30 days during hours of operation. Failure to maintain posting will result in a civil penalty of $100.00 dollars.
