Licensing Program Analyst (LPA) Nolan Tcheng conducted an unannounced 3-Year inspection to the above facility. At 9am, LPA met with Assistant Katia Virabian. Assistant states that licensee just went out to the store. Also present during this inspection is additional Assistant Lyudmila Karamyan. Licensee soon arrived at 9:20am. Licensee states they were at the store. The licensee states that they currently has 12 children enrolled. A current children’s roster is available and is current. Licensee’s hours of operation are 8am-8pm, Monday to Friday.

The house is located on the same property as facility #198021379. This facility is located in the back of the property with a separate address as #198021379. Families ring doorbell on the driveway gate and the provider comes out to receive child to walk to the facility. This is a one-story home which consists of 3 bedrooms, 2 bathrooms, kitchen, living room, front yard and back walkway (fenced). The children use all bedrooms, bathroom in bedroom 1, kitchen (walkthrough), living room, and front yard. Per licensee, areas off limits to children and parents include: bathroom in nap bedroom. All areas identified on the facility sketch were inspected. The licensee provides food for children in care.

The licensee states that 1 adult and 0 minors currently live in the home. Licensee states that they currently have three assistants. All individuals present in the home have obtained a criminal record clearance or exemption prior to working, residing or volunteering in a licensed home. Per licensee, there are no weapons, firearms or bodies of water on the premises.

At 9:20am, LPA was led on a tour of the facility by the licensee. Emergency Disaster Plan, Parent’s Rights Poster and the Facility License are observed to be posted outside front door. Upon entry into the home, the living room was reviewed. There are age appropriate toys and equipment present for children in care. There were 9 children observed eating breakfast. Tour continued through the kitchen. LPA observed sharp knives to be kept in a high cabinet out of reach. Detergents, cleaning compounds, medications, and other items which could pose a danger are inaccessible to children by safety latch on the cabinet under the sink.

At 9:35am, a fire extinguisher was observed in the laundry room attached to the kitchen. The valve on the required 2A 10BC fire extinguisher indicates fully charged and was serviced on 08/26/2024, as indicated on service tag. Per State Fire Marshall standards, fire extinguishers shall be serviced annually. Smoke and carbon monoxide detectors were tested and are operable.

The next room reviewed was the first bedroom, accessible through the kitchen. There is a little ball pit and age appropriate equipment for children. The bathroom that children currently use is attached. LPA reviewed the bathroom and observed it to be free of hazards. Safety latched was on the cabinet under the sink. Tour continued down the hallway. The first bedroom on the left is currently used as the naproom. LPA observed 8 small beds and 3 play yards for children in care to use. Nap equipment was observed to meet safety standards. Licensee's personal closet is present in the naproom and contains personal clothing. The bathroom used by licensee for personal use was observed to have a safety handle on the doorknob to prevent access to children in care.

The bedroom at the end of the hall is considered off limits and has a safety handle to prevent access but was reviewed due to licensee stating it will be used as an isolation room for a sick child. There is a couch that pulls out to create an adult size bed. This is the licensee's personal bedroom. The licensee states that there are no poisons in the home and understands that storage areas for poisons must be locked with a key or combination lock. The licensee states that a cell phone is used and stays at the facility during operating hours.

At LPAs observed the front yard area. The outdoor play area was observed to be fenced. At this time, children are using the front yard for outdoor play time. There are small train and carousel rides that are electric powered. Licensee provided Plan of Use created for the rides and also Instruction packets for each ride. Licensee states the children use it occasionally. The licensee is observed to be operating within the license capacity limitations.

The licensee and other personnel have completed training on preventive health practices including Pediatric First Aid and CPR. The licensee's Pediatric First Aid and CPR expires on 07/2024. LPA discussed that licensee must maintain a current Pediatric First Aid and CPR. Licensee and personnel have completed mandated reporter training that 06/04/2026. The licensee does have proof of immunization against influenza, tuberculosis, pertussis, and measles. Children’s records were reviewed, including but not limited to, a copy of the emergency information card that contains all the information specified by regulation.

There are currently two children present at the facility that have just started as of yesterday 09/03/2024, per Licensee. LPA confirmed they started as of date provided. Licensee states they have provided the enrollment forms to parents and they are bringing them back today. An Advisory Note is being provided to address the missing documents.

The following was discussed:


-There are no pets on the premises.
-Infant Walkers, Johnny Jumpers, Saucer Chairs, and/or any other item that falls into these categories are not permitted in a family child care facility.
-Smoking is prohibited in a license family child care home.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for repeat violation, for maximum of 30 days per person will be assessed if the regulation is violated

At this time, Licensee states they are only looking over two infant age children who are part time and leave each day at 12:30-1pm. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Based on the LPA’s observations and records review, the following deficiencies listed on the attached LIC 809 (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safety.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the licensee Armine Yeghikyan, at 11:47am.
