1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 03/22/24 Licensing Program Analyst (LPAs) Staicy Perry and Claudia Guangorena conducted an unannounced 3yr. Annual Inspection to the above facility. Upon arrival at 8:15am, Licensing staff met with Interim Center Manager Maria Hernandez. Shortly after Ana Marquardt Associate Director of Education arrived and took over the tour. This is an Infant Program with a Toddler Option Program. This facility has (2) Infant classrooms and (2) toddler classrooms. Per Ana, there are currently 8 infants enrolled and 8 Toddlers in the Toddler Option. Hours of operation for this facility are Monday through Friday from 7:30 AM until 2:00 PM. Licensing staff observed a teacher at the front entrance door signing in children as they get dropped off. Per Center Manger all children receive a daily health check upon arrival once child is cleared and after hand washing parents may leave their child. Licensing staff was lead on a tour of the facility. Licensing staff observed all required forms/publications to be posted by the front entrance. Food menus were reviewed to ensure that they are being posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days are available upon request. Facility provides breakfast, lunch and PM snacks. Facility uses CACFP food program. PHYSICAL PLANT: All areas were identified on the Facility Sketch were inspected. The following classrooms were inspected: Classroom 1 (Infant ages 18- 24 months), Classroom 2 (Infant ages 18- 24 months), Classroom 3 (Toddlers ages 24-36 months), and Classroom 4 (Toddlers 24- 36 months). LPAs observed the Toddler classrooms to be physically separate from the Infant classrooms. Individual storage space was observed available for each child inside the classrooms. Toys were observed to be clean and safe. Furniture and equipment were inspected for age appropriateness and good repair. There are smoke detectors and fire extinguishers located throughout the facility. Facility is also equipped with cameras that facility has live and recorded access to at all times. Licensing staff observed the facility to have 3 toilets and 3 sinks available to children. All toilets and sinks are in safe and sanitary operating conditions. Licensing staff observed each classroom to have a changing table within arm’s reach of a sink. Page 1 of 5

The facility was observed to have diapers and wipes available in each classroom near the changing table.Teacher-child ratios were observed to be in accordance with Title 22 regulations. The Licensee is within the conditions, limitations, and capacity specified on the license. LPAs reminded Lead Teacher that all children need to be observed to be under visual supervision of a teacher at all times. Napping equipment (mats/cots) and bedding were inspected for good condition, appropriate storage, and cleanliness. Children's napping sheets and blanket are kept in child’s cubbies as well and is washed on a weekly basis by the facility and as needed. Washer and dryer were observed to be in storage room in hallway near the children’s restrooms made inaccessible to the children. Storage for children's belongings were inspected. All toilets and washing facilities are in safe and sanitary operating conditions. Facility was observed to have one staff bathroom with cleaning solutions that is placed high up.

All classrooms were observed to and inspected for health and safety. First Aid supplies are available and complete in each classroom. Drinking water was readily available indoors in a blue Rubbermaid jug with small disposable cups that are accessible to the children.

At this time, the main office is used as an isolation area, there is a cot available for an ill child to rest and a staff restroom nearby. LPAs observed that main office has a refrigerator for medication if needed to be refrigerated.

Licensing staff observed disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Medication will be placed in a locked container with a latch inside each classrooms Emergency Bags. Classroom furniture and equipment are in good condition, free of sharp, loose, or pointed parts. All floors are clean and safe. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids. The facility was observed to be free of flies, other insects, and rodents.

Licensing staff observed the valve on the required 2A10BC fire extinguisher indicates fully charged and was serviced on August 22,2023, as indicated on service tag. Per State Fire Marshall standards, fire extinguishers shall be serviced annually. Facility has functioning carbon monoxide detectors in eachclassrooms and smoke detector that meets statutory requirements. Center Manager states there are no weapons, firearms, or bodies of water on the premises.

OUTDOORS: Licensing staff reviewed the outdoor play area. Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, swings, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. There are 2 outside play space that is separated by a small gate. One play yard if for infants and the play yard 2 is for the toddlers. LPAs informed Administrator Ana that toddler program shall be conducted in areas physically separate from those used by older or younger children. Play area 1 has 1 play structure with a manufacture label indicating ages 6-23 months of age, bikes were observed as well as lounge furniture that is age appropriate and playhouses for children in care. Outside play space 2 was observed to have patio furniture which were age appropriate and several sensory tables. Drinking water is readily available outdoors in a gallon Igloo container with small cups available for the children while outside play.

FACILITY RECORDS: Sign in and out sheets were reviewed and are placed inside each classroom at the entrance of for parents to sign in and out daily to ensure that the person who signs the child in and out uses their full legal signature and records the time of the day. Disaster drills have been conducted at least every six months. Last drill conducted was March 13, 2024.

There is at least one person trained in CPR and Pediatric First Aid present during this inspection. Licensing staff also reviewed staff records. The review of Staff records was documented on the LIC 859. Staff present had proof of the AB 1207 Mandated Reporter Training certificate on file. Staff present had proof against TB, measles, pertussis, and influenza.

Children’s Records were reviewed for completeness; Inspection of required forms was made and documented on the LIC 857.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Licensing staff discussed the safe sleep regulations with Lead Teacher and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Lead Teacher of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

Licensing staff referred director to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.
Facility has tested for lead 10/11/2022, and had no exceedances. LPAs did observe lead results on parent board.

Ana was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Ana was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative.
