An announced pre-licensing site inspection was conducted today by Licensing Program Analyst (LPA) Crystal Green to inspect and measure facility for capacity determination. Applicant is seeking to obtain licensure for a new preschool program. Due to COVID- 19 precautionary measures were taken, individuals present during inspection wore appropriate personal protective equipment. Licensing staff met with Director, Katherine Clemons, who guided analyst on a tour of the facility. This facility is located on the premises of the Inside Out Church of Los Angeles.

As a COVID- 19 precautionary measures, Director states that parents drop off and pick up will be conducted at the entrance of the parking lot gate. Temperature checks will be conducted, and staff will administer pre-screening questions to all visitors. At 10: 35 am, Licensing staff was guided through a tour of the proposed designated preschool classrooms. This facility will have (4) classrooms. Furniture and equipment were inspected for age appropriateness and good repair. LPA observed classroom 1 and classroom 2 to contain age appropriate furniture and equipment. Individual storage space was observed available for children inside the classroom. Classroom 3 and Classroom 4 was observed to contain furniture and equipment not age appropriate for preschool children. Per Director, classrooms 3 and 4 will not be accessible to children until age appropriate furniture is obtained. LPA advised Director that prior to using classroom 3 and 4 to notify the department.

LPA observed the facility to have a total of 5 toilets, 2 urinals and 4 sinks available to children. All toilets and sinks shall be in safe and sanitary operating conditions. LPA observed one of the two sinks located in the restroom label boys to not be secured to the wall.

There are smoke detectors and fire extinguishers located throughout the premises. There is a fully equipped first aid kit available in classroom 1 and 2. The isolation area for an ill child will be located in the office.

Licensing staff was guided through the proposed outdoor activity space. LPA observed several sticky thorn plants located in areas designated for children. Per Director, plants will be removed from areas designated for children. The outdoor play structure was observed to be needing child safety step stool to ensure children can access the equipment without assistance. LPA observed outdoor grills that will need to be removed from the outdoor activity space. The outdoor area containing the play structure was observed to be missing a fencing enclosure to secure the outdoor activity play area. Per Director, the facility will install a four feet barrier to enclose the outdoor activity play area. Facility will need to install a canopy to provide additional shade for the children while outdoors. All areas around or under high climbing equipment, and slides shall have a cushion material to absorb a fall. Report Continues Page 2 of 4

Facility will provide AM and PM snacks and parents will provide lunch for their child. Per Director, facility will obtain a water cooler with disposable cups for each classroom. No transportation is being conducted by the facility.

Incidental Medical Services (IMS) policy was discussed. For IMS information, see Evaluator Manual- Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following corrections will need to be made prior to submitting application for final approval. Correction will be due by 12/31/2021.

· Applicant will need install a fencing to enclose the designated outdoor activity play area.

· Applicant will need to install a canopy to provide additional shade around the outdoor play structure.

· Applicant will need to remove sticky plant from the outdoor areas designated for the children.

· Applicant will need to obtain age appropriate equipment for the play structure located in the outdoor activity area.

· Applicant will need to obtain age appropriate furniture and equipment for classrooms 3 and 4.

· Applicant will need to repair light fixture located in Classroom 3.

· Applicant will need to repair or replace the sink located in the restroom label boys

· Applicant will need to obtain child safety step stool for the 2nd sink located in the restrooms

Based on measurements obtained today from both indoors and outdoors the capacity for the facility will be limited to classroom 1 and classroom 2 until corrections are received for classrooms 3 and classroom 4. The Classroom 1 and 2 measured for a capacity of twenty (20) preschoolers. Once proof of correction is received and reviewed for classroom 3 and 4, a recommendation will be made to license the above facility for a total capacity of forty-two (42) preschoolers.

An exit interview was conducted with the Director, Katherine Clemons and a copy of this report was provided.
