LPA conducted an unannounced case management inspection for the purpose of interviewing Licensee, Tina Rike regarding a complaint at another facility. LPA informed Licensee of the reason for the visit. There were 13 children present in the green room with two staff and 18 children in the blue room with three staff.

Exit interview conducted with Licensee, Tina Rike. A copy of this report along with the notice of site visit were provided. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
