25 | PRELICENSING INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an announced pre-licensing inspection to the above facility on 10/29/2021. LPA arrived at the facility at 12:45 PM and met with Silvana Zadoorian, Applicant who guided analyst on a tour of the facility. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
Per applicant operation hours will be Monday to Friday, 8:00 a.m. to 6:00 p.m.. Applicant states she will care for children 0-12 years old. For Large Family Child Care Home Fire Clearance was granted on 10/21/2021. Applicant was previously was licensed at 639 W. Arden Avenue, Glendale, CA 91202 with facility number 198018439.
All areas identified on the facility sketch were inspected. This is a one story home. The home consists of 3 bedrooms, 2 restrooms, living room (daycare area), dining room, kitchen, front yard, backyard (fenced), detached converted garage which consist of two rooms and one bathroom.
Areas that are accessible to children are as follows: Living room (daycare area), one bedroom, one bathroom in the hallway, front part of backyard (fenced). Per applicant, parents will enter the home through the main entrance which leads the the living room and dining room.
Areas off limits based on facility sketch submitted to children and parents include: Dining room, master bedroom, bathroom in the master bedroom, one bedroom (TV room), kitchen, back of backyard, and detached converted garage. LPA observed a gate was installed between dining room and living room to make the dining room inaccessible. LPA observed a safety gate was installed to the kitchen to make the kitchen inaccessible. Per applicant, children will only walk through the kitchen to go to the backyard.
**Rooms that are off-limits need to be made inaccessible during operating hours**
The applicant does understand that licensing staff may have access to off-limit areas during inspection visit if necessary.
