25 | On 5/7/2026, at 9AM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an Unannounced Annual/Random inspection to the above facility. Upon arrival, LPA met with Licensee / Hasmik Perikhanyan, who assisted wit the visit. LPA disclosed the purpose of the visit. Entrance Checklist – Family Child Care Homes was provided to the Licensee. Licensee guided the LPA on a tour of the facility. Adults in the home were discussed and all have criminal record clearance. Also present was the Licensee’s husband / Artur Perikhanyan and Assistant / Lusine Soghomonyan Nersisyan. There were five (5) day care children present during today’s inspection. Licensee states that there are currently nine (9) children enrolled. The children's roster was reviewed and is current. Per licensee, the facility’s hours of operation are Monday - Friday, from 8AM - 12AM. Emergency Disaster Plan, License, and Parents’ Rights were posted at the time of inspection. Disaster drill log was also available during today’s inspection and the last disaster drill was conducted on 1/15/2026.
This is a single story home which consists of 4 bedrooms, 2 bathrooms, living room , dining room, kitchen, hallway which leads to bedroom 2 and bathroom 2, front yard, backyard (fenced), a drive way, side yard and a detached garage. In addition to the main dwelling unit, the property includes a separate dwelling unit, with its own address and entrance. The separate residence is a detached structure located in the back of the main dwelling (behind the garage) and functions as an independent living space. The address of the rear dwelling is 823 B Windsor Road, Glendale, CA 91205.
Per licensee, parents drop off and pick up their children directly at the main door, which leads to the living room. Areas that are accessible to children are as follows; Living room, dining room, kitchen, bedrooms 1, 3 and 4, bathroom 1 (located near the kitchen), and back yard (play area). Per Licensee, off-limits areas are as follows: bedroom 2, bathroom 2, hallway which leads to bedroom 2 and bathroom 2, front yard, side yard, driveway, detached garage, and separate dwelling unit in the back. The off-limits rooms are made inaccessible by child proof door knobs/locks. Staff escort children through bedroom 3 to access the backyard. The washer and dryer are located near the kitchen area (between the kitchen and bedroom 3).
