This is a single story home which consists of 4 bedrooms, 2 bathrooms, living room , dining room, kitchen, hallway which leads to bedroom 2 and bathroom 2, front yard, backyard (fenced), a drive way, side yard and a detached garage. In addition to the main dwelling unit, the property includes a separate dwelling unit, with its own address and entrance. The separate residence is a detached structure located in the back of the main dwelling (behind the garage) and functions as an independent living space. The address of the rear dwelling is 823 B Windsor Road, Glendale, CA 91205.

Per licensee, parents drop off and pick up their children directly at the main door, which leads to the living room. Areas that are accessible to children are as follows; Living room, dining room, kitchen, bedrooms 1, 3 and 4, bathroom 1 (located near the kitchen), and back yard (play area). Per Licensee, off-limits areas are as follows: bedroom 2, bathroom 2, hallway which leads to bedroom 2 and bathroom 2, front yard, side yard, driveway, detached garage, and separate dwelling unit in the back. The off-limits rooms are made inaccessible by child proof door knobs/locks. Staff escort children through bedroom 3 to access the backyard. The washer and dryer are located near the kitchen area (between the kitchen and bedroom 3).

• Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.

• Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

LPA observed the back yard has artificial grass and adequate perimeter fencing throughout the yard. The LPA advised the Licensee that it is best practice to ensure outdoor supervision is provided at all times. LPA observed age appropriate toys free of loose and sharp parts available for the children in the backyard and all trees, shrubs, and plants are maintained. Per licensee, there are no pets and LPA did not observe any pets at the time of visit. The facility is equipped with surveillance cameras located inside bedroom 3 and outside in the back yard / play area. The Licensee has three firearms on the premises. The firearms are secured in a locked safe located within an off-limits bedroom and are inaccessible to children. The firing pins have been removed from both firearms and are stored in a locked location away from the firearms. Ammunition is also stored separately and secured in a locked container apart from the firearms. Per Licensee, there are no bodies of water around the premises.

The valve on the required 2A 10BC fire extinguisher indicates fully charged (last serviced on 11/3/2025). Smoke and carbon monoxide detectors were tested and observed to be in operable condition. Per Licensee, the First Aid kit is kept on the wall, near the washer in dryer. The First Aid kit was inventoried for necessary supplies. The Licensee does not have a current Pediatric First Aid and CPR. Proof of immunization against influenza, pertussis, and measles were readily available during today’s inspection. The Mandated Reporter Training for the Licensee is current.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Rooms that are off-limits need to be made inaccessible during operating hours. NO smoking, NO infant walkers, NO Johnny jumpers, NO saucer chairs, NO incline sleepers and any other item that falls into that category are permitted in the facility. Effective January 1, 2010, licensees of family child care homes are required to ensure that at least one staff member with current training in pediatric first aid and pediatric CPR is on site at all times when children are present.

LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep web page at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA reviewed and issued the LIC 311D - Forms/Records to Keep in Your Family Child Care Home. Mandatory Forms for the children’s files and staff files, requirements for fire drills, earthquake drills and documentation were discussed. Role and responsibilities of being a Mandated Reporter were reviewed. The Licensee was advised how to access forms and Regulations online at www.ccld.ca.gov. Licensee was made aware that it is his/her responsibility to know the regulations as well as anyone who assists in providing care.

AB 1207: Beginning on January 1, 2018, this law requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. During the exit interview, the Licensee confirmed that there are no Registered Sex Offenders living in the facility.

No deficiencies were cited during today's inspection. However, the LPA issued an advisory note and requested corrective action.

A notice of site visit was given and must remain posted for 30 days.
