25 | REQUIRED- 1 YEAR INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced required 1- year and case management- licensee initiated inspections to the above facility on 06/09/2023 due to the licensees are requesting capacity increase. LPA arrived at the facility at 8:25 AM and met with Marine Lalayan and Amo Hovanisian, Licensees who guided analyst on a tour of the facility. A COVID 19 risk assessment was conducted prior to entering the facility. Per Marine Lalayan and Amo Hovanisian, Licensees facility hours are Monday to Saturday from 6:00 a.m. to 11:00 p.m. LPA provided a copy of Entrance Checklist—Family Child Care Home (LIC126) to licensees.
Licensees have been licensed at this address since 06/02/2022. Fire clearance for Large Family Child Home was granted on 06/08/23.
This is a one story home located on the first level. The home consists of 3 bedrooms, 3 & 1/2 bathrooms, kitchen, dining room, living room (FIREPLACE: inaccessible) (main day care area), family room, basement (gym), detached garage, front yard and backyard (fenced). Per licensees the children use the living room (daycare classroom), family room, one bedroom adjacent to the family room, one bathroom in the daycare bedroom, and daycare outdoor activity area in the backyard. Per licensees areas off limits to children and parents include: Dining room, kitchen, two bedrooms, one bathroom in the master bedrooms, basement (gym), bathroom in the basement, detached garage , bathroom in the detached garage, front yard, and off-limit area in the back yard. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report.
At 8:50 am, the licensees began touring LPA through the home starting with the entry way which lead directly to the living room (daycare classroom), The living room was inspected and electrical outlets were observed to be covered.
