25 | Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an announced Case Management – Licensee Initiated inspection at the above referenced facility on October 31, 2025. The purpose of the visit was due to the licensee’s request for a capacity increase. LPA arrived at the facility at 12:30 PM and met with Licensee / Julieta Ghazaryan, who guided the LPA on a tour of the facility. Adults in the home were discussed and all have criminal record clearance. Also present were, Licensee’s daughter and husband. There were 3 day care children present during today’s inspection. Licensee states that there are currently 3 children enrolled. The children's roster was reviewed and is current. Per licensee, the facility’s hours of operation are Monday - Friday, from 6AM - 11:30PM. Emergency Disaster Plan, License, and Parents’ Rights were posted at the time of inspection. Disaster drill log was also available during today’s inspection and the last disaster drill was conducted on 7/7/25. Fire Clearance for Large Family Child Care Home was granted on 10/13/2025.
This is a single story home which consists of 2 bedrooms, 2 bathrooms, living room, dining room, kitchen, laundry room and front yard. There is a separate dwelling located at the front of the property, functioning as an independent living space with its own address: 564 W Harvard St, Glendale, CA 91204.
According to the Licensee, parents drop off and pick up their children by entering through the driveway, walking passed the front unit, and proceeding through the gate that leads to the main entrance of the family child care home. Areas that are accessible to children are as follows: Living room, dining room, bedroom 1, bathroom 1 and front yard. The home does not have a fireplace. The front yard has artificial grass with age appropriate toys. Per Licensee, off-limits areas are as follows: Bedroom 2, bathroom 2, kitchen and laundry room. The off-limits rooms are made inaccessible by child proof door knobs/locks and child safety gates.
