On 07/17/2024 at 11:20 am, Licensing Program Analyst (LPA), Crystal Green conducted an unannounced Case Management inspection for a change in capacity. Upon arrival, LPA met with Licensee Representative, Shannon Gale. This facility is currently licensed to serve 58 preschool children ages 2 to 6 years old in the green, yellow, blue classrooms and the Play Lab. The Licensee, Spark Preschool LLC., is requesting to reduce the existing preschool capacity to 50 preschoolers serving ages 2 to 6 years old in classroom blue and green classrooms and the Play Lab; and add a new toddler option component with a capacity of 8 toddlers serving ages 18 to 36 months in the yellow classroom; the overall capacity will remain at 58 children. Hours of operation will be Monday to Friday from 7am to 6pm.

The facility is located on the premises of the Temple Knesset Israel. Per Director, the church does not operate during school hours. There is a designated pickup and drop off area for parents will be located off La Mirada which is the back of the building. The preschool has its own entrance separate from the Temple.

Present during today’s inspection were 33 children with 5 staff. Appropriate ratios and supervision were observed. All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Shannon Gale that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities.

When medications are on site, Shannon Gale stated that they will be stored in an inaccessible cabinet within each child’s designated classroom. A fully equipped first aid kit is located in throughout the building. There is an operational carbon monoxide detector on site located in each classroom. All required licensing documents were observed posted near the entrance of the facility. Children will be signed in and out at the front and rear of the building using the Bright wheel app.

LPA continued to tour the facility and obtained the measurements for the new toddler indoor activity space. The total indoor activity space for the toddler option program measured 288.33 square footage, which is sufficient to accommodate the requested capacity of 8 toddler children. The preschool program indoor activity space will remain at 2,052.10 square footage, which is sufficient for the existing licensed capacity of 50 preschoolers.

LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via filter water dispenser with disposable cups. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 6/11/2024.

In the yellow room the toddler classroom, there is a changing table with at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink. LPA observed a total of 1 sink and 1 toilet available for the toddlers to use located across the hall from the blue classroom. The Preschool children will use the restrooms located near the Play Lab. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the staff office area located near the Play Lab.

Facility will provide AM/PM snacks. Parents provide their child’s lunch. The kitchen area currently includes microwave, full size refrigerator, and prep counter. The kitchen area and food storage areas were observed to be free of rodents and/or vermin. Hazardous items in kitchen are inaccessible to children via the door leading to the kitchen area with be keep closed and locked.

The facility currently has one fully fenced playground area. Fencing is made of a plywood which was observed to be at least four feet high. The total square footage for all the outdoor activity space for the preschool is 2,236.66 square feet. This facility has an approved waiver on file to allow staggered use of the outdoor playarea to obtain the requested preschool capacity. At this time, the toddlers do not have a designated outdoor play area. A waiver to share the playground at different times in the AM and PM due to the toddlers not having their own yard shall be submitted and approved prior to licensure.

Shade is provided via canopies and fixed shade structure. There are sufficient outdoor age-appropriate toys and play equipment available on the playground(s). There is a climbing structure on the preschool playground for children ages 2 to 5 years which is properly anchored. There is adequate cushioning in fall zones of climber provided by outdoor surface mats and artificial turf. Drinking water is available via filtered water dispenser. LPA observed all hazardous items on the playgrounds to be inaccessible to children. Applicant was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

During this inspection, LPA obtained a waiver request letter and pictures of the outdoor play area. The Licensee Representative, Shannon Gale, was informed that the waiver request will be reviewed by the Regional Office for final approval in order to obtain the capacity for the toddler option component.

Upon a final file review and approval of the outdoor waiver request, a license for 58 children (50 preschoolers and 8 toddlers) will be granted.

Exit interview conducted and report was reviewed with the Licensee Representative, Shannon Gale.

A notice of site visit was given to Licensee Representative and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.
