PLAN OF CORRECTION INSPECTION CONDUCTED IN ARMENIAN
Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced Plan of Correction inspection to the above facility on 03/19/25. LPA arrived at the facility at 1:40 PM. LPA entered the facility through the back gate which leads to the playground. LPA met with Adrineh Khanmalekian and Lyubov Ghaltakhchyan, licensee’s assistant. Upon LPA’s arrival to the facility licensee was not present in the home. LPA talked to licensee, Margarit Tumasyan over the phone who stated she stepped out for a Doctor appointment and will take her long to get home.
The following corrections were observed during this inspection:

- LPA observed child proof safety latches on the cabinets in the bathroom to make cleaning compounds and personal hygiene inaccessible to the children.


- During this inspection children roster was reviewed and completed.
- During this inspection LPA review 4 out of 12 children files and were completed.
- Per licensee, children use the bathroom, and potty chair will be used for potty training only. During this inspection LPA observed potty chair in the hallway of entrance and asked licensee to keep the potty chair in the bathroom effective today and empty the potty chair after each use.
Acknowledgement forms for 03/06/35 inspection was observed for all children in care.
LPA cleared deficiencies on this date and provided a copy of the Licensing Report Licensee's assistant and issued POC clearance letter.
At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today from the areas that were inspected today.
A notice of site visit was given and must remain posted for 30 days.
Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
LPA Eivazian cleared deficiencies that were issued on 03/06/25.
Exit interview conducted and report was reviewed with the licensee's assistant, Adrineh Khanmalekian..
