25 | On 9/11/25, at 9:30AM, Licensing Program Analyst (LPA) Joe Katrdzhyan conducted an Unannounced Required - 3 Year inspection to the above facility. Upon arrival, LPA disclosed the purpose of the inspection and met with Licensee, Ofelya Sargsyan who guided the LPA on a tour of the facility. Adults in the home were discussed and all have criminal record clearance. Also present were, Licensee’s daughter and husband. There were 9 day care children present during today’s inspection. Licensee states that there are currently 14 children enrolled. The children's roster was reviewed and is current. Per licensee, the facility’s hours of operation are Monday - Sunday, from 4AM - 11PM. Emergency Disaster Plan, License, and Parents’ Rights were posted at the time of inspection. Disaster drill log was also available during today’s inspection, last disaster drill conducted on 8/9/25.
This is a single story home which consists of 3 bedrooms, 3 bathrooms, living room, dining room, kitchen, a detached converted garage (used for activities), front yard and backyard. Per Licensee, during pick-up and drop-off, parents enter the facility through the main entrance which leads to the living room and side gate/driveway which leads to the backyard. Areas that are accessible to children are as follows: Bedrooms 1 and 2 (which are adjacent to the living room), bathroom 1, bathroom 2 (which is located inside of bedroom 2), living room, dining room, kitchen, detached converted garage, and backyard. The living room has a fireplace which has been screened to prevent access by children. Per Licensee, the fireplace will not be used. The back yard has artificial grass with age appropriate toys. Per Licensee, off-limits areas are as follows: Bedroom 3 (master bedroom), bathroom 3 (which is located inside of bedroom 3), and front yard. The off-limits rooms are made inaccessible by child proof door knobs/locks. The washer/dryer are located inside the kitchen.
All areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for the safety of the children. There is a working telephone
