An announced pre-licensing site inspection was conducted today by Licensing Program Analyst (LPA) Crystal Green to inspect and measure facility for capacity determination. Licensing staff met with Applicant, Wesley Lewis, who guided analyst on a tour of the facility. This facility is located on the same premises as Christ Center Church. Proposed operating hours will be Monday through Friday from 10:00 AM to 3:00 PM.

At 10:22 AM, Licensing staff was guided through a tour of the proposed School-Age designated areas. This located has access to 2 floors. Parents and children will enter facility from Isabel Street via the parking lot. Per Applicant, there will be a sign in sheet located at the side door.

Facility will have a total of 6 Rooms, kitchen, and dining area. The First Floor will have 3 classroom areas (Science RM, Garden RM, and Sanctuary Area). The Second Floor will have 3 classrooms area (RM 1, RM, and Studio RM). Furniture and equipment were inspected for age appropriateness and good repair. LPA observed an area rug located on the second floor in RM 2 to be a possible tripping hazard. Individual storage space will be available for each child. Drinking water will be made available via a water jug and disposable cups. Page 1 of 4

 LPA observed the facility to have restroom accessible to the children located on the 1st floor which contains 3 toilet, 1 urinal and 4 sinks. LPA observed a restroom available on the 2nd floor which contains 1 toilet and 1 sink. Per Applicant, staff will provide supervision to ensure 1 child uses the restroom at a time if needed. All toilets and sinks are in safe and sanitary operating conditions.

The Studio RM will serve as an isolation area for an ill child and ill children will use the staff bathroom. There is a first aid kit available on site. There are smoke detectors and a carbon monoxide detector on site that meets statutory requirements. This facility has a kitchen on site. Parents will provide meals and snacks for enrolled children. Food preparation areas were inspected for cleanliness. Per Director, the kitchen area will also serve as a home economics room. Licensing staff observed the proposed outdoor activity to currently belong to a licensed preschool (198014948). Per Director, facility received permission to share usage of the outdoor space. The school-age program does not have a separate designated outdoor area. LPA advised Director to request a waiver to share usage of the outdoor space to ensure the yard is not used at the same time as the preschool children. The outdoor play was observed to have tables available along with canopy shade. Transportation will not be provided by the facility.

Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must

obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual- Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA reviewed with facility representative the LIC 311A, Records to Be Maintained at The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform.

An approved Fire Clearance has been received by the Department. Based on measurements obtained today, capacity for the school-age will be seventy- five (75) children. LPA consulted with the applicant regarding information needed to submit a waiver request for the share usage of the outdoor play area. LPA informed applicant that waivers are not automatically granted and will need to be review by the Regional Office for final approval. LPA obtained pictures during this inspection.
