An announced pre-licensing site inspection was conducted today by Licensing Program Analyst (LPA) Crystal Green to inspect and measure facility for capacity determination. The applicant is requesting is requesting to operate a Preschool Program. Licensing staff met with Applicant, Victoria Zargaryan. The proposed hours of operation will be Monday to Friday from 8 am 5:30pm.

The entrance to the facility will be accessible from Kentworth St., parents will enter the parking lot from California Ave. and exit onto Kentworth St. Per Applicant, staff will be located in the parking lot to supervise drop offs and pickups.

At approximately 10:40am, LPA was provided a tour of the facility. This facility has 2 floors. Per Applicant, the second floor will be off-limits to parents and children. The facility will have a total of 8 classrooms and 1 auditorium located on the first floor. LPA observed that children will have access to restroom located outdoors. The outdoor restroom was observed to have 5 toilets, 2 urinals, and 4 sinks available to children. LPA observed 3 classrooms located near Playyard 1. Furniture and equipment were inspected for age appropriateness and good repair. Individual storage space was observed located within the classrooms. Additional classrooms were observed available to children within the building on the first floor. There are 5 classrooms located indoors. LPA observed the facility to have additional restrooms available to children. The restrooms contained 4 toilets, 2 urinals, and 4 sinks. All toilets and sinks are in safe and sanitary operating conditions. There are smoke detectors and fire extinguishers located throughout the premises. The isolation area for an ill child will be located in the office. Per Applicant, the facility will utilize the Brightwheel app to communicate with parents and for sign in and sign outs. There is also a fully equipped first aid kit and a locking medication cabinet located in the office.
Licensing staff was guided through the proposed outdoor activity space. There will be 2 play areas available for children in care. The designated outdoor play area was observed to be enclosed by a fence. LPA did not observe outdoor playground equipment during this inspection.

Facility will provide snacks and lunch to the children in care. There will be a kitchen on site. LPA observed the kitchen area to be in process of completion. No transportation will be conducted by the facility.

Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

The Department has received an approved Fire Clearance. Based on measurements obtained today, the capacity for the preschool will be one hundred and nine (109) children.

The following correction needs to be made before receiving final approval. Corrections will be due by 04/15/2023.

• The Applicant will submit proof of the outdoor play areas containing playground equipment, cushioned surface, and adequate shading.
• The Applicant will submit proof of the facility's completed interior classrooms
• The Applicant will submit proof of (2) barriers installed to prevent access to the 2nd floor.
• The Applicant will submit proof of the completed kitchen area.

At approximately 2:30pm, an exit interview was conducted with Applicant and a signed copy of this report was provided.
