On 11/5/2025, at 9:00 am , Licensing Program Analysts (LPAs) Shushanik Safaryan and Crystal Green conducted an unannounced visit to deliver complaint fndings for the complaint received by the Department.
During the visit,LPAs met with Facility Representatives ,Annette Clark and Suzie Johnson to whom the purpose of the visit was explained.During this visit , LPA observed 15 infants and 4 satff members.

Complaint alleged: Staff did not follow proper reporting requirements.

Per the Reporting Party (RP) ,a child fell from the playground steps, and no report was made. The LPA was unable to interview RP to obtain additional information regarding the above allegation.

During this investigation , LPA toured the facility and interviewed seven staff members and parents.

During interviews, facility representatives stated that incident (ouch) reports are created by teachers and submitted to the Department in accordance with Reporting requirements by an assistant Director.
Facility representatives also reported that no incidents had occurred in the playground area. No disclosures were made by staff members or parents to support allegations above.

Complaint alleged: Unqualified staff provide care and supervision for children in care.

Per Reporting Party staff #1 and staff #6 missing required units for providing care to infants .During this investigation , LPA interviewed staff members and reviewed staff files. Files observed complete and had required units and experience.

Complaint alleged: Bedding is shared by different children in care.

During this investigation, LPA toured the facility and observed extra clean sheets available in the napping room . LPA observed cribs labeled with an infant’s names and noted that laundry machines are located within the facility. The Facility Representative stated that sheets for infants are washed and changed daily. LPA continued the tour into the toddler room and observed cots labeled with children names, with additional sheets stored in the designated storage area. According to the Facility Representative, toddler sheets are washed twice a week or daily if needed. During the staff interviews , all staff members confirmed that the facility provides sheets, which are washed daily for infants and either twice a week or as needed for toddlers. Staff reported that during closing , one staff removes used sheets and replaces them with clean ones and starts laundry process. The morning staff member removes sheets from the dryer , folds, and stores them. All staff members denied that bedding is shared among different children without laundering first. No disclosures were made by staff to support allegations.

Complaint alleged Staff do not check infants every 15 minutes as required.

During this investigation , LPA conducted interviews with 7 staff members. Per Facility Representative , 15 minute checks are conducted every day by staff members and submitted to Assistant Director by the end of the month. Statement was confirmed by staff members. Additionally ,15- minute check forms were provided to LPA for review. LPA reviewed 15- minute check forms for children enrolled for the months of October.

Based on the investigation, although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is unsubstantiated.
Exit interview conducted with Facility Representative, Suzie Johnson and Annette Clark and Appeal Rights explained.

The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.

During the visit,LPA met with Facility Representative ,Annette Clark and Suzie Johnson to whom the purpose of the visit was explained.During this visit , LPA observed 15 infants and 4 satff members.
Complaint alleged: Staff did not provide adequate supervision for children in care. Per RP,infants were left alone with doors closed in nap area.

During this investigation , LPA toured the facility on 10/08/2025 and observed nap area separated from the activity area by a wall and a door . LPA noted that staff members in the activity area are unable to visually observe children in the nap area.

LPA interviewed seven staff members . During the interviews, staff disclosed that there are not always staff members present in the nap area. Additionally , when infants nap door is kept closed. According to the assistant director, staffing in the nap area depends on the schedule and staff availability .The Director stated that there is a monitor in the nap area :however, during the visit, LPA and the director observed that the monitor was not operational.

Complaint alleged: Hazards are accessible to children in care. Per Reporting Party hot beverages are accessible to the children .

During this investigation , LPA interviewed staff members. Per Director ,she observed staff members having drinks in the rooms and were advised to keep it inaccessible to the children in kitchen area. Staff members stated they are allowed to have water or drinks with closed lids. Some disclosed beverages are allowed as long they are on the counter in the kitchen area within classroom. LPA observed kitchen area separated from the classroom with the gate which was open during the visit. Per staff members gate open when snacks are served.

Complaint :Staff did not provide adequate food service for children-in-care. Expired, moldy and unlabeled snacks were provided for children.

During this investigation , LPA touted the facility and inspected cabinets where snacks and food were stored . During the tour , LPA observed 3 cans of infant formula on the top of the cabinet with an expiration date 07/22/2025. According to the Facility Representative , the formulas were donated to the facility and were not intended for use , as parents of children provide their own formula. Next LPA toured Turtles classroom kitchen area and observed unopen pack of corn puffs best by 10/06/2025. LPA Inspected cabinets and observed containers with Cheerios stored inside without labels . A staff member stated that when food items are removed from their original packaging, staff are required to label. The staff member further stated she observed labels prior. It was disclosed that the staff member previously responsible for snack handling and storage is no longer employed by the facility.

Complaint alleged :Staff did not maintain proper teacher child ratios. Per RP ratios not consistently maintained, especially Monday, Wednesday and Friday.

During today’s visit, upon arrival LPAs Shushanik Safaryan and Crystal Green observed 5 infants with an Assistant Director and parent with their own child in the Koalas room . No other staff member observed in the room . Two minutes later another staff member came in from downstairs. Per Facility Representative and staff members , facility was having picture day downstairs and staff members were assisting rest of the infant’s downstairs for picture. Staff members stated they were notified more infants arrived to the class and one staff member came back to the classroom.

Based on LPA`s observations and interviews which were conducted and record reviews, the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12 , Chapter 1 ) are being cited on the attached LIC 9099 D.
