25 | On 09/04/2026, Licensing Program Analyst (LPA) Shushanik Safaryan conducted an unannounced POC (Plan of Correction) inspection to the above facility to ensure Type A deficiency cited on 08/28/2026 have been cleared.
Upon arrival LPA met with Annette Clark, assistant principal and Shannon Lyons-Heberger, Business Manager. Tour was provided.
During this inspection, LPA observed 6 infants with 3 staff members, 3 infants with one staff member in the nap room and 7 toddlers with 2 staff members.
During this visit, LPA toured the facility and observed all required postings in the front visible to the children`s representatives, LPA observed signed Acknowledgement of Receipt of Licensing Report (LIC 9224) for each child enrolled.
Prior to the visit, facility submitted Infant Safety, Supervision, and Handoff Policy and Licensing visit Meeting agenda which was reviewed with staff on 08/31/2026 and signatures were obtained.Further, facility will follow up with Technical Support Program for additional training.
During the visit, LPA cleared deficiencies cited on 08/28/2026 and provided a copy of the Licensing Report to Licensee and issued POC clearance letter.
The Notice of Site Visit (LIC 9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
